Dr. John F. Ryan (1935 - ), Associate Professor of Anaesthesia at the Harvard Medical School, influenced the careers of hundreds of residents and fellows-in-training while instilling in them his core values of resilience, hard work, and integrity. ⋯ Although he had had many accomplishments, he identified his experiences caring for patients with malignant hyperthermia and characterizing the early discovery of this condition as his defining contribution to medicine. Based on a series of interviews with Dr. Ryan, this article reviews a remarkable career that coincides with the dawn of modern pediatric anesthetic practice.

Randomized Controlled TrialThe effects of bispectral index monitoring on hemodynamics and recovery profile in developmentally delayed pediatric patients undergoing dental surgery.
General anesthesia is often preferred for dental surgery or rehabilitation in developmentally delayed pediatric patients. Bispectral index monitoring is used to monitor the depth of anesthesia and to ensure early recovery. However, studies on the topic in developmentally delayed pediatric patients are limited. ⋯ In conclusion, results suggest that routine BIS monitoring may be beneficial due to its favorable effects on the recovery profile in developmentally delayed pediatric patients.

Dexmedetomidine has established a firm position in the armamentarium of anesthesia pharmacology. However, it is still a relatively new drug and its application is based on early evidence that is intriguing but far from conclusive. Based on experience with previous cure-alls, anesthesia clinicians and researchers must insist on more information and formulate appropriate science that will allow us to truly understand the role of this dexmedetomidine in our specialty—as wonder drug or also-ran.
