Severe traumatic brain injury (TBI) is a major cause of morbidity and mortality globally. The Brain Trauma Foundation guidelines advocate for the maintenance of a cerebral perfusion pressure (CPP) between 60 and 70 mmHg following severe TBI. However, such a uniform goal does not account for changes in cerebral autoregulation (CA). ⋯ CPPopt represents an alternative target for cerebral haemodynamic optimisation following severe TBI, and early observational data suggest improved neurological outcomes in patients whose CPP is more proximate to CPPopt. The recent publication of a prospective randomised feasibility study of CPPopt guided therapy in TBI, suggests clinicians caring for such patients should be increasingly familiar with these concepts. In this paper, we present a narrative review of the key landmarks in the development of CPPopt and offer a summary of the evidence for CPPopt-based therapy in comparison to current standards of care.

Midodrine is a peripherally acting, oral α-agonist that is increasingly used in intensive care units despite conflicting evidence for its effectiveness. It has pharmacological effects on blood vessels as well as pupillary, cardiac, renal, gastrointestinal, genitourinary, lymphatic and skin tissue. It has approval for use as a treatment for orthostatic hypotension, but a surge in interest over the past decade has prompted its use for a growing number of off-label indications. ⋯ Early retrospective studies supported its use for this indication, but more recent randomised controlled trials have largely refuted this practice. Key questions remain on its role in managing critically ill patients before intensive care admission, during intensive care stay, and following discharge. This narrative review presents a comprehensive overview of midodrine use for the critical care physician and highlights why lingering questions around ideal patient selection, dosing, timing of initiation, and efficacy of midodrine for critically ill patients remain unanswered.

The College of Intensive Care Medicine of Australia and New Zealand is responsible for credentialling trainees for specialist practice in intensive care medicine for the safety of patients and the community. This involves defining trainees' performance standards and testing trainees against those standards to ensure safe practice. The second part examination performed towards the end of the training program is a high-stakes assessment. ⋯ There is increasing expectation for medical specialist training colleges to provide fair and transparent assessment processes to enable defensible decisions regarding trainee progression. Examinations are a surrogate marker of clinical performance with advantages, disadvantages and inevitable compromises. This article evaluates the Hot Case examination using Kane's validity framework and van der Vleuten's utility equation, and identifies issues with validity and reliability which could be managed through an ongoing improvement process.

Background: Persistent psychological distress occurs frequently in family members of patients who die in an intensive care unit (ICU). Objective: To determine the effectiveness of bereavement interventions in reducing persisting psychological distress in bereaved family members after death in an adult ICU. Design: Systematic review and meta-analysis of studies that assessed the effect of bereavement interventions on persisting psychological distress in bereaved family members of ICU patients. ⋯ Scores for Impact of Event Scale, Impact of Event Scale-Revised and Inventory of Complicated Grief were measured in some but not all studies. There was no effect of an intervention on HADS scores (weighted mean difference, -0.79 [95% confidence interval, -3.81 to 2.23]; I2 = 65.8%). Conclusions: Owing to limited data, and clinical and statistical heterogeneity, there is considerable uncertainty regarding whether bereavement support strategies reduce, increase or have no effect on psychological distress in bereaved family members.

Objective: Non-invasive thermometers are widely used in both clinical practice and trials to estimate core temperature. We aimed to investigate their accuracy and precision in patients admitted to the intensive care unit (ICU). Study design: Systematic review and meta-analysis. ⋯ Risk of bias for the index test was unclear, mostly because of no device calibration or control for confounders. Conclusions: Compared with the gold standard of intravascular temperature measurement, non-invasive peripheral thermometers have low accuracy. This makes their clinical and trial-related use in ICU patients unreliable and potentially misleading.
