Intestinal obstruction caused by omphalomesenteric duct remnant: usefulness of laparoscopy.
The anomalies related to omphalomesenteric duct remnant constitute an uncommon cause of intestinal obstruction, of which Meckel"s diverticulum and its variants represent the most important clinical presentation. In most cases they are asymptomatic and usually affect young patients. ⋯ Treatment is surgical and consists in exeresis of the diverticulum or the fibrous band causing the clinical picture. We report two cases of persistence of the vitelline duct resolved by laparoscopic approach.

Failure of endoscopic therapy in upper gastrointestinal hemorrhage due to duodenal ulcers.
to evaluate the efficacy of endoscopic treatment in patients with upper gastrointestinal (UGIH) due to duodenal ulcer with high risk of persistent or recurrent bleeding and to determine the associated failure factors of this procedure. ⋯ endoscopic therapy is an efficient treatment of upper gastrointestinal bleeding due to duodenal ulcer. By means of a clinical variable, the hemodynamic status and two endoscopies, bleeding stigmata and the size of the ulcer, a group of patients with high risk of endoscopic treatment failure can be selected.

to assess whether a magnetic resonance cholangiopancreatography performed after an acute biliary pancreatitis leads to pancreatic morphological alterations and if secretin stimulation influences the visualization of the pancreatic tree. ⋯ acute biliary pancreatitis leads to morphological alterations, regarded as scar lesions which do not become chronic. Secretin stimulation improves the visualization of the main pancreatic duct.

Acute pancreatitis (AP) is a serious disease with a frustrating mortality rate, but with a very good quality of life reported among survivors, that justifies an optimised allocation of therapy intensity. ⋯ In our Institution (a tertiary hospital) AP diagnosis is quickly made, local and systemic complications are clearly diagnosed and monitored, but at least 50% of patient waited for 2 days until ICU admission, representing those who die earlier.
