Unplanned intubation during anesthesia: review of 31 cases from the Thai Anesthesia Incidents Study (THAI study).
To examine the causes, outcomes, and contributing factors associated with patients requiring unplanned emergency intubation for adverse respiratory events. METERIAL AND METHOD: Appropriate unplanned intubation incidents were extracted from the Thai Anesthesia Incidents Study (THAI Study) database conducted between February 1, 2003, and January 31, 2004, and analyzed using descriptive statistics. ⋯ Major incidents of unplanned intubation occurred after bronchoscopy. Common contributing factors related to inadequate ventilation, airway obstruction, sedative agents and unstable hemodynamics. Quality assurance, additional training, and improved supervision tended to minimize the incidents.

The limited protocol MRI in diagnosis of lumbar disc herniation.
To assess agreement in detection of lumbar disc herniation (LDH) between limited and full protocol MRI. ⋯ The authors concluded that limited protocol MRI may replace full protocol MRI in diagnosis of LDH but not in nerve root compression.

Randomized Controlled Trial Comparative Study
Pioglitazone reduces urinary protein and urinary transforming growth factor-beta excretion in patients with type 2 diabetes and overt nephropathy.
Increased urinary excretion of protein and transforming growth factor-beta (TGF-beta) are associated with progression of diabetic nephropathy (DN). Thiazolidinediones (TZD) could reduce urinary protein excretion in patients with microalbuminuric DN. There is little data of patients with macroalbuminuric DN. Also, there are no available clinical data regarding the effect of TZD on TGF-beta and type IV collagen in clinical DN. The present study was carried out to evaluate the effect of pioglitazone (PGZ), a member of TZD, on urinary protein, urinary TGF-beta, and urinary type IV collagen excretion in type 2 diabetic patients with macroalbuminuric DN. ⋯ Besides the effectiveness in blood sugar control, pioglitazone could salutarily reduce proteinuria and synthesis of TGF-beta as well as type IV collagen. These beneficial effects of pioglitazone on diabetic nephropathy are comparable to angiotensin converting enzyme inhibitors and angiotensin receptor blockers

Differences in socio-economic status, service utilization, and pregnancy outcomes between teenage and adult mothers.
Teenage pregnancies put mothers at high-risks to many health-related complications and newborn infants to poor birth-outcomes. ⋯ The prevalence of teenage pregnancies in Thailand remains high. Most teenage mothers and their newborn infants are vulnerable to a variety of potentially serious health problems, and accordingly need appropriate help and support.

Anxiety, a psychological response to stress, can affect a child undergoing medical procedures, and their family members. The purpose of the present study was to evaluate the benefit of detailed systematic information using a set of real photograph-based technical illustrations provided to both parents and patients before performing endoscopy on the level of anxiety. ⋯ The preparatory intervention using systematic visual illustration of the technical procedures in children undergoing gastrointestinal endoscopy could significantly reduce the parents' anxiety.
