Smoking causes about 5 Million deaths worldwide every year. Prevention and cessation of smoking should therefore be one of the most important public health priorities. ⋯ The continuous empathic support and motivation by the family doctor is however the most important factor. Even a small advance within this step-by-step strategy can be considered as a success which will be followed by further progress.

Randomized Controlled Trial Multicenter Study Comparative Study[Long-term efficacy of dapagliflozin in patients with type 2 diabetes mellitus receiving high doses of insulin].
Dapagliflozin, a selective inhibitor of sodium-glucose cotransporter 2, may improve glycemic control with a lower dose of insulin and attenuate the associated weight gain in patients with inadequate control despite high doses of insulin. ⋯ Dapagliflozin improves glycemic control, stabilizes insulin dosing, and reduces weight without increasing major hypoglycemic episodes in patients with inadequately controlled type 2 diabetes mellitus.

Comparative Study[Implementation of the German S3-Guideline of non-invasive ventilation for acute respiratory failure in daily practice].
Application of non-invasive ventilation (NIV) in acute respiratory failure (ARF) since publication of the German S3 guideline in 2008. ⋯ The S3 guideline for NIV in ARF is well-known in intensive care in Germany, but not all are aware of the detailed re-commendations. The use of NIV in ARF has increased since publication of the guideline but nearly always follows additional clinical assessment in decision-making. In one third it is restricted to special indications, with very limited application in non-cardiac hypoxemic ARF. Full-face masks are preferred according to the guideline. All ICUs, that participated in the survey, hold ventilators for NIV.
