Review Meta AnalysisIs Extracorporeal Membrane Oxygenation the Standard Care for Acute Respiratory Distress Syndrome: A Systematic Review and Meta-Analysis.
Acute respiratory distress syndrome (ARDS) is a type of acute respiratory failure syndrome characterised by severe respiratory distress and stubborn hypoxaemia. Patients with ARDS have a prolonged hospital stay and high mortality rate. Over long-term follow-up, ARDS is found to be associated with a high incidence of long-term complications and decreased quality of life. Venovenous extracorporeal membrane oxygenation (vv-ECMO) has been widely used for the treatment of refractory ARDS. However, it is not the standard treatment as recommended by ARDS guidelines. ⋯ Compared with CMV, ECMO contributed to lower 60-day and 1-year mortality, and increased ICU mortality in patients with ARDS. However, H1N1 ARDS was independently associated with higher ICU mortality and nosocomial pneumonia. The results were not affected by removing retrospective control studies or articles published >20 years ago from the sensitivity analysis. This meta-analysis demonstrates the effectiveness of ECMO and its importance in standard treatment of patients with ARDS.

Meta Analysis Comparative StudyUnilateral Versus Bilateral Antegrade Cerebral Perfusion: A Meta-Analysis of Comparative Studies.
Antegrade cerebral perfusion (ACP) is an essential adjunct for prolonged hypothermic circulatory arrest (HCA) during aortic arch surgery. However, it has yet to be established whether ACP should be delivered unilaterally or bilaterally. The aim of the present meta-analysis is to investigate outcomes of unilateral ACP (uACP) compared to bilateral ACP (bACP) in comparative studies. ⋯ For patients undergoing aortic arch surgery, the available evidence supports either uACP or bACP as an adjunct to HCA. However, there is insufficient comparative evidence available to determine the benefit of either modalities in patients with longer durations of circulatory arrest.

Review Meta AnalysisHeart Valve Surgery Performed by Trainee Surgeons: Meta-Analysis of Clinical Outcomes.
Cardiac surgical units must balance trainee education with the duty to provide optimal patient care. This is particularly challenging with valvular surgery, given the lower volume and increased complexity of these procedures. The present meta-analysis was conducted to assess the impact of trainee operator status on clinical outcomes following valvular surgery. ⋯ Valvular surgery cases performed primarily by trainees were not associated with adverse perioperative outcomes. These findings suggest the rigorous design of cardiac surgical trainee programs can sufficiently mitigate trainee deficiencies. However, studies with longer follow-up duration and echocardiographic data are required to assess long-term durability and safety.

Review Meta AnalysisThe Efficacy of Danshen Injection as Adjunctive Therapy in Treating Angina Pectoris: A Systematic Review and Meta-Analysis.
During the last 40 years, Danshen injection has been widely used as an adjunctive therapy for angina pectoris in China, but its efficacy is not yet well defined. The objective of this study was to verify the efficacy of Danshen injection as adjunctive therapy in treating angina pectoris. ⋯ This study showed that Danshen injection as adjunctive therapy seemed to be more effective than antianginal agents alone in treating angina pectoris. However, more evidence is needed to accurately evaluate the efficacy of Danshen injection because of the low methodological quality of the included RCTs.

Review Meta AnalysisAntiarrhythmics in Cardiac Arrest: A Systematic Review and Meta-Analysis.
It is widely accepted that antiarrhythmics play a role in cardiopulmonary resuscitation (CPR) universally, but the absolute benefit of antiarrhythmic use and the drug of choice in advanced life support remains controversial. ⋯ This systematic review and meta-analysis suggests that, based on current literature and data, there has been no conclusive evidence that any antiarrhythmic agents improve rates of ROSC, survival to admission, survival to discharge or neurological outcomes. Given the side effects of some of these agents, we recommend further research into their utility in current cardiopulmonary resuscitation guidelines.
