Meta AnalysisRole of the ultrasonographic 'whirlpool sign' in intestinal volvulus: a systematic review and meta-analysis.
Intestinal volvulus is a potentially life-threatening condition that occurs when loops of bowel twist around its supporting mesentery and associated vasculature. Clinicians often rely on various radiological investigations for prompt diagnosis to avoid complications such as bowel infarction. This review assesses the clinical reliability of the ultrasonographic whirlpool sign (WS) in the diagnosis of intestinal volvulus. ⋯ Though the pooled sensitivity was less than ideal (87.42%), this review and meta-analysis nevertheless supports the reliability of the ultrasonographic WS as an acceptable indicator of intestinal volvulus.

Meta AnalysisSystematic review of disparities in surgical care for Māori in New Zealand.
Health equity for Indigenous peoples in the context of surgery has recently become topical amongst surgeons in Australasia. Health inequities are amongst the most consistent and compelling disparities between Māori and New Zealand Europeans (NZE) in New Zealand (NZ). We aimed to investigate where ethnic disparities in surgical care may occur and highlight some of the potential contributing factors, over all surgical specialties, between Māori and NZE adults in NZ. ⋯ Despite the significant variation in the types of diseases, procedures and indicators of surgical care of the included studies, consistent findings are that disparities in different aspects of surgical care exist between Māori and NZE in NZ. This review highlights the need to better quantify the important issue of health equity for Māori in surgery given the lack of studies over the majority of surgical specialties.

Review Meta Analysis Comparative StudyIntravenous lignocaine in colorectal surgery: a systematic review.
Colorectal surgery leads to morbidity during recovery including pain and fatigue. Intravenous (IV) lignocaine (IVL) has both analgesic and anti-inflammatory effects that may improve post-operative pain and recovery. The aim of this review is to compare the effectiveness of IVL to other perioperative analgesia regimens for reducing pain and opioid consumption following colorectal surgery. ⋯ IVL has shown limited benefit towards reducing early pain and morphine consumption when compared with placebo in colorectal surgery. However, IVL did not show any significant reduction in pain or opioid consumption when compared with epidural. Further research investigating IVL combined with intraperitoneal local anaesthetic is warranted.

Review Meta Analysis Comparative StudyMeta-analysis of two surgical approaches for calcaneal fractures: sinus tarsi versus extensile lateral approach.
There remains a controversy between extensile lateral approach and sinus tarsi approach (STA) to treat calcaneal fractures. The aim of this study is to evaluate the medium- and long-term outcomes of the STA versus extensile lateral approach in terms of intraoperative variables, quality of alignment on a lateral radiograph incidence of wound healing complication, visual analogue scale and American Orthopaedic Foot and Ankle Society for the treatment of calcaneal fractures. ⋯ The existing evidence supports STA to be a better approach for treating calcaneal fractures, and this might aid in the management of this injury.

Review Meta Analysis Comparative StudyComparison of outcomes between minimally invasive oesophagectomy and open oesophagectomy for oesophageal cancer.
A systematic review with meta-analysis was performed to compare perioperative outcomes between minimally invasive oesophagectomy (MIE) and open oesophagectomy (OE). ⋯ Regarding certain clinical outcomes, MIE may be more beneficial than OE.
