Acute Intraoperative Hyperkalemia During Robot-Assisted Radical Cystectomy: A Case Report.
A 50-year-old man with muscle-invasive bladder cancer was scheduled for a robotic radical cystectomy. Four hours into the surgery, his electrocardiogram showed rhythm disturbances. Arterial blood gas analysis showed a serum potassium concentration of 6.6 mEq/L. ⋯ The procedure was completed after conversion to an open surgery. The postoperative serum potassium concentration was reduced to 4.6 mEq/L, and the patient was extubated. The remainder of his hospital stay was uneventful.

Thoracic epidurals remain the optimal method for providing postoperative analgesia after complex open abdominal and thoracic surgeries. However, they can be challenging to both place and maintain, as evidenced by a failure rate that exceeds 30%.1 Proper identification of the epidural space and accurate placement of the catheter are critical in order to deliver effective postoperative analgesia and avoid failure.2,3 This case series investigated the difficulty in correctly identifying the proper vertebral level for thoracic epidural catheter procedures when performed in the lateral decubitus position.

Effect of Combined Epidural Morphine and Midazolam on Postoperative Pain in Patients Undergoing Major Abdominal Cancer Surgery.
The objective was to compare analgesic effect of combined epidural morphine-midazolam with either drug alone on postoperative pain in patients undergoing major abdominal cancer surgery. ⋯ Addition of midazolam to epidural morphine prolonged time of first analgesic request and decreased total analgesic consumption.

Postoperative hemodynamic deterioration among cardiac surgical patients can indicate or lead to adverse outcomes. Whereas prediction models for such events using electronic health records or physiologic waveform data are previously described, their combined value remains incompletely defined. The authors hypothesized that models incorporating electronic health record and processed waveform signal data (electrocardiogram lead II, pulse plethysmography, arterial catheter tracing) would yield improved performance versus either modality alone. ⋯ Clinical deterioration prediction models combining electronic health record data and waveform data were superior to either modality alone, and performance of combined models was primarily driven by waveform data. Decreased performance of prediction models during temporal validation may be explained by data set shift, a core challenge of healthcare prediction modeling.

The past, the present and the future of machine learning and artificial intelligence in anesthesia and Post Anesthesia Care Units (PACU).
Over the past decade, artificial intelligence (AI) has largely penetrated our daily life. Hence, our expectations regarding clinical AI are very high. However, in healthcare and especially in perioperative medicine, the impact of AI is still relatively limited. ⋯ If successfully implemented and integrated into the clinical workflow, AI-assisted perioperative medicine could become more preventative and personalized. However, AI implementation is not the final step. New challenges will follow implementation including algorithm maintenance, continuous monitoring, and improvement.
