Review Meta AnalysisNear infrared spectroscopy for frontal lobe oxygenation during non-vascular abdominal surgery.
Cerebral deoxygenation, as determined by near infrared spectroscopy (NIRS), seems to predict postoperative complications following cardiac surgery. We identify the type of non-vascular abdominal surgery associated with cerebral deoxygenation and/or hyperoxygenation, how such deviations affect patient outcome, and whether maintained cerebral oxygenation improves outcome. ⋯ NIRS seems important for predicting neurological complications associated with LTx. Also, surgery in reverse Trendelenburg's position and in other types of abdominal surgery about one-fourth of the patients are subjected to episodes of cerebral deoxygenation that seems to predict a poor outcome. Although there are currently only few studies available for patients going through abdominal surgery, the available evidence points to that it is an advantage to maintain the NIRS-determined cerebral oxygenation.

Multicenter Study Comparative StudyOptimizing Survival of Patients With Marginally Operable Stage IIIA Non-Small-Cell Lung Cancer Receiving Chemoradiotherapy With or Without Surgery.
For marginally operable stage IIIA non-small-cell lung cancer (NSCLC), surgery might not be done as planned after neoadjuvant concurrent chemoradiotherapy (CCRT) for reasons (unresectable or medically inoperable conditions, or patient refusal). This study aims to investigate the outcomes of a phased CCRT protocol established to maximize the operability of marginally operable stage IIIA NSCLC and to care for reassessed inoperable patients, in comparison with continuous-course definitive CCRT. ⋯ For marginally operable stage IIIA NSCLC, our phased CCRT strategy may optimize survival by maximizing operability and maintain an acceptable survival for reassessed inoperable patients by split-course CCRT boost following maintenance chemotherapy.

Spinal bupivacaine is used for day-case surgery but the appropriate dose that guarantees hospital discharge is unknown. ⋯ Ambulatory surgery is possible under spinal anaesthesia with bupivacaine although the dose range that ensures reliable anaesthesia with duration short enough to guarantee ambulatory management is narrow.

Endoscopic skull base surgery is becoming more popular as an approach to the anterior skull base for tumors and cerebrospinal fluid (CSF) fistulae. It offers the advantages of better cosmesis and improved quality of life after surgery. We reviewed the complication rates reported in the literature. ⋯ The endoscopic approach is an increasingly accepted technique for anterior skull base tumor surgery and is associated with acceptable complication rates. Increasing experience with this technique can decrease rates of complications.
