This review summarizes the role of soluble guanylate cyclase (sGC)-cyclic guanosine 3', 5'-monophosphate pathways in heart failure and several new drugs that modify guanylate cyclase. The sGC activators and stimulators as modulators of sGC are promising drugs in the therapy for decompensated heart failure and pulmonary hypertension. ⋯ Riociguat is an NO-independent direct stimulator of sGC with beneficial effects in patients with decompensated heart failure and pulmonary hypertension. The sGC modulators play an important role in patients with heart failure and pulmonary hypertension.

ReviewSurgery in older cancer patients - recent results and new techniques: worth the investment?.
Recent developments in oncogeriatric surgery focus on several items - preoperative risk estimation and identification of frail patients and optimalization of perioperative care. New screening tools are being evaluated and show promising results. ⋯ The recent trend towards individualized treatment schemes will certainly be of benefit for the elderly population. More tools are becoming available to answer the most difficult question of all, namely whether surgery is the optimal treatment in this individual frail elderly oncogeriatric patient.

ReviewPain management of pancreatic head adenocarcinomas that are unresectable: celiac plexus neurolysis and splanchnicectomy.
Pancreatic adenocarcinoma is often incurable at the time of diagnosis. For patients with unresectable or recurrent disease, palliation of pain is a key component of care. Medical management with narcotics has numerous side effects and may be ineffective. Interventions for pain control include celiac plexus neurolysis (CPN) and splanchnicectomy. The purpose of this review is to outline pertinent anatomy, techniques, side effects, complications, and efficacy of interventions for palliation of pain from pancreatic cancer. ⋯ Patients with incurable pancreatic carcinoma should be offered either CPN or splanchnicectomy when medical management with narcotics has failed.

ReviewResponding to offers of altruistic living unrelated kidney donation by group associations: an ethical analysis.
Members of a religious community presented to our institution and offered to be altruistic unrelated living kidney donors en masse. This situation raised a number of ethical concerns related to the situation specifically and to altruistic unrelated living kidney donation in general. Based on our ethical analysis, we made the following conclusions. ⋯ Appointing a donor's advocate can facilitate donor understanding of the risks, benefits, and alternatives to organ donation and can help discern coercion. Furthermore, an experienced social worker should assess all potential donors and discern if coercion is present; potential donors should be referred to a psychologist or psychiatrist as appropriate. Although concerns regarding allocation were not raised by our specific situation, ethical concerns regarding allocation of kidneys donated by altruistic unrelated kidney donors exist (eg, requesting that such donations be allocated to recipients with specific characteristics).

Randomized Controlled Trial Comparative Study[Comparison of the effects of BiPAP ventilation combined with lung recruitment maneuvers and low tidal volume A/C ventilation in patients with acute respiratory distress syndrome].
To compare the effects of BiPAP ventilation combined with lung recruitment maneuvers (LRM) with low tidal volume A/C ventilation in patients with acute respiratory distress syndrome (ARDS). ⋯ The results show that combination of LRM with BiPAP mode ventilation, as compared with the control group, contributes to improved FiO2/PaO2 ratio, pulmonary compliance, stable hemodynamic and shorter duration of ventilation support in patients with ARDS.
