Randomized Controlled Trial Comparative StudyInspection on instrument insertion during colonoscopy: a randomized controlled trial.
Polyps seen and not removed during colonoscope insertion are sometimes unable to be found during withdrawal. ⋯ Inspection during colonoscope insertion offered no additional benefit compared with an equivalent period of inspection performed entirely during withdrawal. These results do not support an additional role for routine inspection during colonoscope insertion.

Colonoscopy is used for the detection of neoplastic polyps, although a significant miss rate has been reported. Limited data suggest that the administration of the antispasmodic hyoscine N-butylbromide during colonoscopy improves polyp detection. ⋯ ISRCTN25405865.).

Randomized Controlled TrialStrategies for training in diagnostic upper endoscopy: a prospective, randomized trial.
Training simulators have been used for decades with success; however, a standardized educational strategy for diagnostic EGD is still lacking. ⋯ Structured simulator training supplementing clinical training in upper endoscopy appears to be superior to clinical training alone. Simulator training alone does not seem to be sufficient to improve endoscopic skills.

Randomized Controlled Trial Comparative StudyA prospective, randomized study of EUS-guided celiac plexus neurolysis for pancreatic cancer: one injection or two?
The technique of alcohol injection during EUS-guided celiac plexus neurolysis (CPN) in patients with pancreatic cancer-related pain has not been standardized. ⋯ There were no differences in onset or duration of pain relief when either 1 or 2 injections were used. There was no difference in safety or survival between the 2 groups.

Randomized Controlled Trial Comparative StudyNitrous oxide for analgesia in colonoscopy without sedation.
Colonoscopy is associated with pain and discomfort, and intravenous analgesics and sedatives are widely used. There are several disadvantages regarding this practice, including risk of complications, resources demanded, and amnesia after sedation. In spite of promising results in previous studies, nitrous oxide is rarely used at endoscopy centers around the world. ⋯ Nitrous oxide given intermittently is not an effective substitution for intravenous on-demand sedation and analgesics in the setting of colonoscopy without sedation.
