The incidence of intracranial bleeding is known to be markedly higher in Japan and other East Asian countries than in countries outside of East Asia. Non-vitamin K antagonist oral anticoagulants (NOACs) have much lower risk of intracranial bleeding than warfarin, so we reviewed the effect of this class of drugs on intracranial bleeding in Asian patients with non-valvular atrial fibrillation (NVAF). ⋯ Irrespective of the lower PT-INR of warfarin, the incidence of intracranial bleeding is still high in Asia and East Asia. Because each NOAC strongly reduces the incidence of intracranial bleeding when compared with warfarin, use of dabigatran, rivaroxaban, apixaban or edoxaban would seem the best option for stroke prevention when treating Asian patients, including Japanese with NVAF.

Acute kidney injury (AKI) and acute hyperglycemia are associated with unfavorable outcomes. The impact of acute hyperglycemia on the development of AKI after acute myocardial infarction (AMI), however, remains unclear. This study was undertaken to assess the relationship between admission glucose and incidence of AKI after AMI. ⋯ Admission hyperglycemia might have contributed to the development of AKI in patients with AMI.

Early hemodynamic performance of the trifecta bioprosthetic valve in patients with aortic valve disease.
The Trifecta valve (St Jude Medical) is a novel supra-annular aortic bioprosthesis designed to improve hemodynamic performance. We hypothesized that the Trifecta may offer better hemodynamic performance in Japanese patients, in whom the annulus is smaller, compared with Western populations. We compared the early results of hemodynamic performance between the Trifecta and the Magna (Edwards Lifescience) valves at our institution. ⋯ The Trifecta valve provides excellent early postoperative hemodynamic performance in Japanese patients. Patients with a small annulus size relative to body size may benefit more from the Trifecta in terms of postoperative hemodynamic performance.

Postoperative atrial fibrillation (POAF) is a common complication of cardiac surgery and results in increased health-care utilization. This study identified new transthoracic echocardiographic predictors of POAF using an index of the total atrial conduction time derived on tissue Doppler imaging (PA-TDI duration) in patients undergoing off-pump coronary artery bypass grafting (OPCAB). ⋯ PA-TDI duration was an independent predictor of POAF following OPCAB. Awareness of risk of POAF may lead to the prevention of POAF, a rapid response to POAF, shortened hospital stay, and improved prognosis.

Postoperative cardiac events are an important cause of morbidity and mortality in patients undergoing non-cardiac surgery. Predictive values of surgical parameters with respect to development of postoperative cardiac events have not been well investigated. ⋯ Surgical parameters, including surgery time and transfusion, can strongly predict development of postoperative MACE in patients undergoing non-cardiac surgery.
