To prospectively investigate the predictive value of the preoperative C-reactive protein (CRP) concentration for major postoperative complications following off-pump coronary artery bypass (OPCAB) surgery. ⋯ Elevated preoperative CRP level and/or preoperative CRF indicate increased risk of developing major postoperative complications, particularly acute postoperative renal dysfunction in patients undergoing OPCAB surgery.

Although the metabolic syndrome (MetS) is considered to be caused primarily by visceral fat accumulation, epidemiological evidence is lacking as to whether or not obesity is an essential element in the syndrome. ⋯ MetS caused moderate increases in all-cause and CVD mortality. However, the MetS definition requiring obesity may not necessarily identify non-overweight individuals who have a high mortality risk and are more prevalent than subjects with MetS.

The pathogenesis of out-of-hospital cardiac arrest (OHCA) without organic heart disease has not been fully investigated. ⋯ OHCA has a heterogeneous pathogenesis and so dual induction tests are necessary.

The factors contributing to the efficacy and outcome of direct hemoperfusion using polymyxin-B immobilized fiber (PMX-DHP) after cardiac surgery were investigated. ⋯ Prompt initiation of PMX-DHP with drug treatment during the postoperative course of patients with septic shock caused by systemic inflammatory response syndrome related to infection and who are refractory to vasopressor treatment, can improve the disease state before multiple organ failure develops.

Comparison of arterial blood gases of laryngeal mask airway and bag-valve-mask ventilation in out-of-hospital cardiac arrests.
Focusing on the efficacy of successful ventilation during cardiopulmonary resuscitation (CPR) with alternative airways, previous reports investigated various parameters such as success rate, tidal volume, incidence of regurgitation, etc. However, there are few investigations of arterial blood gases (ABG) during CPR with alternative airways, especially the laryngeal mask airway (LMA). ⋯ LMA does not greatly benefit the respiratory status of patients such as in this study population. Delayed placement of a LMA will be recommended to achieve minimally interrupted chest compression in an out-of-hospital CPR protocol for witnessed VF cases following shock.
