Transfusion-related acute lung injury (TRALI) remains the deadliest complication of transfusion. Consensus definitions of TRALI have been developed but remain controversial. Recent evidence supports a strong relationship between blood transfusion and the development of acute lung injury in the critically ill and trauma population. ⋯ The 'two hit' model may best explain the immune and nonimmune pathogenesis of TRALI. Current treatment remains largely supportive; effective measures for decreasing the incidence of TRALI include the use of predominantly male plasma and apheresis platelets. Greater understanding of the blood component and patient risk factors for TRALI will hopefully lead to novel treatment and preventive strategies for reducing the risk of this life-threatening syndrome.

ReviewProlonged glucocorticoid treatment and secondary prevention in acute respiratory distress syndrome.
Experimental and clinical evidence has demonstrated a strong association between dysregulated systemic inflammation and progression of acute respiratory distress syndrome (ARDS). In ARDS, glucocorticoid receptor-mediated downregulation of inflammation is essential to restore homeostasis and decrease morbidity and mortality. ⋯ Treatment was also associated with a marked reduction in the risk of death (relative risk: 0.68; 95% CI: 0.56-0.81; p < 0.001) and a sizable increase in mechanical ventilation-free days (weighted mean difference: 6.58 days; 95% CI: 2.93-10.23; p < 0.001); and intensive care unit-free days (weighted mean difference: 7.02 days; 95% CI: 3.20-10.85; p < 0.001). We recommend that prolonged methylprednisolone treatment, at an initial dose of 1 mg/kg/day in early ARDS and 2 mg/kg/day in unresolving ARDS, be delivered as an infusion to avoid glycemic variability, and that infection surveillance be strictly implemented to identify infections in the absence of fever.

Malignant pleural mesothelioma is a highly invasive tumor arising from the mesothelial cells of serosal surfaces. Several chemotherapeutic agents have been tested for the treatment of this disease and doublet cisplatin with antifolates has been demonstrated to have significant efficacy in Phase III studies. ⋯ Targeted therapies are being developed in oncology and emerging evidence suggests that they offer disease control in several tumors. This article reviews the knowledge on the malignant pleural mesothelioma molecular pathway and focuses on results of clinical trials conducted on this devastating disease.

ReviewObesity hypoventilation syndrome: therapeutic implications for treatment.
Obesity hypoventilation syndrome occurs in obese individuals who are unable to compensate for the added load of obesity on the respiratory system, with resultant daytime hypercapnia in the absence of other causes of alveolar hypoventilation. Significant morbidity and mortality is seen in this disorder if appropriate treatment is not undertaken. ⋯ However, a paucity of well-designed clinical trials and the absence of data from long-term follow-up means that comparison between various treatment options is not possible. Consequently, evidence-based treatment guidelines are currently lacking.

For the last 100 years, the tuberculin skin test (TST) has been the only diagnostic tool available for latent TB infection (LTBI) and no biomarker per se is available to diagnose the presence of LTBI. With the introduction of M. tuberculosis-specific IFN-gamma release assays (IGRAs), a new area of in vitro immunodiagnostic tests for LTBI based on biomarker readout has become a reality. In this review, we discuss existing evidence on the clinical usefulness of IGRAs and the indefinite number of potential new biomarkers that can be used to improve diagnosis of latent TB infection. We also present early data suggesting that the monocyte-derived chemokine inducible protein-10 may be useful as a novel biomarker for the immunodiagnosis of latent TB infection.
