A significant number of children under 3 years sustain a head injury every year. Despite this few studies have provided detailed information about these injuries. ⋯ Children under 12 months are at significant risk of head injury, many of these injuries could be prevented by increased parental supervision or improved home safety.

To evaluate whether an alteration of the lower limb axis is associated with meniscal extrusion. Materials and Methods: Ninety-four patients who had complained of knee pain with good knee function and had a knee magnetic resonance image (MRI) and a full-length X-ray taken of the lower limb were included in the study. Meniscal extrusion was measured in the coronal MRI. Subluxation of the meniscus was considered minor or physiological if ≤ 3 mm, and major if >3 mm. The extrusion as a percentage of meniscus size was also calculated. Knee alignment (varus, negative value; valgus, positive value) was correlated with the presence of minor and major extrusion. ⋯ In patients complaining of knee pain with good knee function, there is no relationship between mild malalignment and the degree of meniscus extrusion.

Acetabular fractures in the elderly are challenging injuries. The use of a trabecular metal acetabular cage was investigated as the treatment option in a series of elderly patients with acetabular fractures. ⋯ No mechanical failure, screw breakage, loosening, or migration was noticed. This novel indication of the cup-cage construction that uses revision techniques, for selected patients and fractures, to achieve an acute stable reconstruction, should be considered as an alternative reconstruction option in elderly patients presenting with acetabular fractures.

Single dose Gentamicin (240 mg) has been shown to reduce postoperative wound infection in patients with fractured neck of femur when used in a combined antibiotic regimen. However, concerns have arisen about the risk of acute kidney injury (AKI) in these patients. ⋯ Single preoperative doses of Gentamicin (240 mg) have no detrimental impact on renal function in this group. There is, however, a significant rise in creatinine following fractured neck of femur surgery, independent of Gentamicin.

Identifying the differences between infants with non-accidental head injuries (NAHI) and accidental head injuries (AHI) may help alert clinicians to recognize markers of abuse. A retrospective review of infants <1 year of age admitted to a tertiary referral centre in Malaysia over a two year period with a diagnosis of head injury or abnormal computed tomography head scans was conducted to identify the clinical features pointing towards a diagnosis of NAHI by comparing the socio-demographics, presenting complaints, clinical features and the extent of hospital investigations carried out. ⋯ Subdural haemorrhages were more common in NAHI infants. The history, mechanism of injury, presenting signs and symptoms as well as the nature of the injuries sustained are all valuable clues as to whether a head injury sustained during infancy is likely to be accidental or not.
