Several equations have been developed to estimate glomerular filtration rate (GFR). The aim of this study was to evaluate the performance of the three equations derived from Modification of Diet in Renal Disease (MDRD) formula: a-MDRD (original simplified), c-MDRD (modified with the Chinese coefficient), and j-MDRD (modified with the Japanese coefficient) in Chinese patients with chronic kidney disease (CKD). We also established the serum creatinine (Scr) reference intervals for Chinese healthy population. ⋯ No racial differences in the Scr reference intervals were observed. The modified MDRD formulas based on data from Asian population do not significantly improve the accuracy and bias, as compared those of the original MDRD. Therefore, whether the original MDRD for GFR estimation needs to be modified by an ethnic factor requires further investigation and validation.

Acute kidney injury (AKI) is one of the most serious postoperative complications of cardiac surgery. The lack of early and powerful markers for AKI makes the morbidity and mortality still very high. HNL (Human neutrophil lipocalin)/NGAL (Neutrophil gelatinase-associated lipocalin) was recently shown as a novel biomarker for AKI after cardiac surgery. ⋯ We confirmed that urine HNL/NGAL may be a useful early biomarker of postoperative kidney injury. The results indicate that the antibody configuration of the assay has an impact on the clinical performance of the assay.

Congenital Generalized Lipodystrophy (CGL) or Berardinelli-Seip Syndrome (BSCL) is a rare autosomal recessive disease characterized by complete absence of adipose tissue and by several metabolic alterations in carbohydrate (diabetes mellitus) and lipid metabolism and involvement of heart, bone and ovaries. Mental retardation and psychiatric disturbances are present in a variable proportion of affected patients. In the present review, the major advances in clinical, molecular and genetic characterization of BSCL affected subjects are recorded and discussed.

Hydroxocobalamin and cyanocobalamin interference on co-oximetry based hemoglobin measurements.
Hydroxocobalamin (OHCbl) and cyanocobalamin (CNCbl) are dark-red colored analogs of vitamin B-12. OHCbl is used as an alternate antidote for cyanide poisoning. Due to the strong red color, if uncorrected, these cobalamins interfere with hemoglobin measurements and can introduce errors in spectrophotometric assay on co-oximeters. The impact of cobalamins on commonly used co-oximetry systems was compared to evaluate the accuracy of hemoglobin measurements and to further assess the ability of the instruments to detect and flag cobalamin interference. ⋯ Among the 4 co-oximeters tested in this evaluation, GEM Premier 4000 and Rapidpoint 405 analyzer showed minimal impact for hydroxocobalamin concentrations around 0.5 g/l. Cyanocobalamin displayed similar interference effect on co-oximetry measurements as OHCbl. The error detection system in the GEM Premier 4000 appropriately detected and flagged interferences on sample measurements.
