Prospective observational study of delirium recovery trajectories and associated short-term outcomes in older adults admitted to a specialized delirium unit.
To describe the recovery trajectories of delirium and to determine factors predicting the course of recovery and adverse outcome. ⋯ Individuals with residual SSD had prolonged recovery trajectory of delirium. These new insights into the recovery trajectories of delirium may help formulate early discharge planning and provide the basis for future research on delirium treatment.

This article describes the implementation of the Care for Seniors model of care, an innovative approach to improving care coordination and integration, and provides preliminary evidence of effective use of specialist resources and acute care services. ⋯ This model of care represents a promising collaboration between primary care and specialist care for improving care to frail older adults living in rural communities, potentially improving timely access to health care and crisis intervention.

Contrasting effects of geriatric versus general medical multimorbidity on quality of ambulatory care.
To determine whether greater burden of geriatric conditions would have contrasting effects on quality of care (QOC) than nongeriatric, general medical conditions. ⋯ Greater burden of geriatric conditions, or geriatric multimorbidity, is associated with poorer QOC. Geriatric multimorbidity should be targeted for better care using a comprehensive approach.

To understand whether nursing home (NH) introduction of culture change practices is associated with improved quality. ⋯ The introduction of NH culture change appears to result in significant improvements in some care processes and outcomes in NHs with high practice implementation. For other NHs, culture change introduction results in fewer survey deficiencies.

Primary care availability and emergency department use by older adults: a population-based analysis.
To assess the relationship between the number of primary care providers (PCPs) in an area and emergency department (ED) visits by older adults. ⋯ These findings suggest that primary care availability has at most a limited effect on ED use by older adults in North Carolina.
