Injuring the internal carotid artery (ICA) is a feared complication of endoscopic endonasal approaches. ⋯ The proposed endoscopic classification outlines key anatomic reference points independent of the vessel's geometry or the sinonasal pneumatization, thus serving as (1) a practical guide to navigate the ventral cranial base while avoiding injury to the ICA and (2) further foundation for a modular access system.

Multimodal intracranial monitoring in the neurosurgical patient requires insertion of probes through multiple craniostomies. ⋯ Hummingbird Synergy is a novel single-port access device for multimodal intracranial monitoring that can be placed safely at the bedside or in the operating room with placement accuracy and has a complication profile similar to or better than that for standard external ventricular drains.
