Transcranial Doppler (TCD) ultrasonography is an important tool for noninvasive detection and monitoring of vasospasm and other pathological conditions of the intracranial vessels. ⋯ Image guidance can be applied easily and efficiently to TCD. It provides anatomic orientation and may help to standardize investigation protocols, define pathological vascular territories for repeat investigations, and thus reduce interinvestigator variations. Image guidance may also extend the use of TCD to situations of a pathological or variant vascular anatomy.

Intraoperative rupture of an intracranial aneurysm is a potentially devastating but avoidable and manageable complication of aneurysm surgery. ⋯ Intraoperative rupture of an intracranial aneurysm is a potentially devastating complication, particularly if a tear occurs at the neck. This simple yet effective method has been very useful in repairing a partial avulsion or tear of the neck of an aneurysm.

There is substantial evidence to suggest that oxidative stress plays a significant role in the development of acute brain injury after subarachnoid hemorrhage (SAH). ⋯ These findings suggest that nesfatin-1, which appears to have antiapoptotic and anti-inflammatory properties, exerts neuroprotection in SAH-induced injury in rats by inhibiting neutrophil infiltration and subsequent release of inflammatory mediators.
