Although various models of ethical decision-making in nursing have been developed, these models only identify the steps for ethical decision-making but describe no framework within which the decisions must be made. This model for ethical decision-making in nursing describes the practice of the nurse/midwife, steps for ethical decision-making as well as the framework for ethical decision-making. This model can be used as a guideline facilitating ethical decision-making which can be justified and explained.

Because of the many problems relating to health care delivery in Africa, it is becoming apparent that neither the exclusive/monopolistic nor the tolerant legislative systems should be tolerated any longer. Especially since the Alma Ata Conference held by the WHO/UNICEF there has been growing impetus towards either inclusive/parallel (the beneficial co-existence of traditional and modern medical systems), or integrated systems. Although the idea of making traditional and modern medicine work in tandem in a united treatment context has its merits, it is also plagued by issues such as the nature of the products of an integrated training, resistance by stubborn protagonists of either of the two systems, or that only lip-service is paid to the idea of co-operation. Nevertheless, it is believed that all interest groups--the authorities responsible for health care delivery, the Western-trained health care workers, the traditional healers and the users of these services--stand to gain from such liaison.

Teenage pregnancy is one of the many public health problems facing the community in Bophuthatswana and neighbouring areas. Health professionals have attempted to address the problem yet little has been done to determine the role of the adolescent male in the prevention of this community problem. This study addresses the male adolescent's sexual behaviour, his attitude towards contraception, contraceptive use and premarital pregnancy. ⋯ The study further revealed that parents did not discuss teenage pregnancy and contraception with their children, instead this subject was discussed among friends at school. Respondents expressed fear when asked why the subject was never discussed with parents, some actually stated that their parents would 'flog' them if they initiated the subject on sex and related matters. The urgent need for formalized sex education in Bophuthatswana was expressed by 77% of the respondents.

Epidural anaesthesia as a method of pain relief during labour has lately become very popular. Statistics show that in some labour units up to 70 per cent of all patients undergo epidural anaesthesia. The popularity of this method can be attributed to its effectiveness in relieving pain during labour. ⋯ It has also been considered whether protocols in the maintenance of epidural anaesthesia exist. All the abovementioned information was acquired by submitting a questionnaire for completion by midwives practising in labour wards in the Johannesburg area. The results of this research show that the midwife's knowledge as regards to epidural anaesthesia and its maintenance is inadequate.(ABSTRACT TRUNCATED AT 400 WORDS)
