Randomized Controlled TrialInstillation of 5% Povidone-Iodine Ophthalmic Drops Decreases the Respiratory Rate in Children Undergoing Strabismus Surgery: A Randomized Controlled Trial.
To investigate the effects of topical application of ophthalmic 5% povidone-iodine eye drops, which has been reported to cause apnea in spontaneously breathing children during general anesthesia. ⋯ Topical application of 5% povidone-iodine eye drops causes a slowing and pause in spontaneous ventilation in a majority of children prior to strabismus surgery. Randomized Controlled Trial Comparative StudyClonidine premedication versus placebo: effects on postoperative agitation and recovery time in children undergoing strabismus surgery.
Postoperative agitation is common in adults and children following the use of several anesthetics, particularly inhalation anesthetics. This behavior has detrimental effects both physically for the patient following the procedure and psychologically for the parent or guardian. The authors propose that clonidine, an alpha-2 agonist, would provide a reduction in children's postoperative agitation and, in turn, improve parent satisfaction in children undergoing strabismus surgery. ⋯ Children receiving clonidine prior to undergoing strabismus surgery have a small but noticeable reduction in postoperative agitation, stay slightly longer in the post-anesthesia care unit, and have higher rates of parent satisfaction.

Randomized Controlled TrialThe effect of different doses of ketamine on intraocular pressure in anesthetized children.
When used for induction of anesthesia, ketamine usually increases intraocular pressure (IOP). However, the effect of low doses of ketamine, which are used for parental separation in children, is unknown. We studied the effect of two different doses of ketamine on IOP in anesthetized children. ⋯ In children anesthetized with halothane, ketamine had a dose-dependent effect on IOP, with 6 mg/kg of the drug causing a small increase in IOP at 5 to 10 minutes and 3 mg/kg not altering the IOP. The higher dose of ketamine also was associated with an increased incidence of postoperative complications.

Randomized Controlled Trial Comparative Study Clinical TrialComparison of mydriatic regimens used in screening for retinopathy of prematurity in preterm infants with dark irides.
To determine the mydriatic regimen that provides optimal dilation of the pupil with minimal systemic side effects for screening of retinopathy of prematurity. ⋯ The prepared combination of cyclopentolate 0.2% + phenylephrine 1% appears to be the mydriatic of choice for preterm infants with dark irides as it provided adequate pupillary dilation with the least systemic side effects.

Randomized Controlled Trial Comparative Study Clinical TrialComparison of ondansetron with ondansetron plus dexamethasone for antiemetic prophylaxis in children undergoing strabismus surgery.
Children undergoing strabismus surgery have a high incidence of postoperative nausea and vomiting. Ondansetron plus dexamethasone is effective in reducing its incidence in many surgical procedures. ⋯ Ondansetron and ondansetron plus dexamethasone were equally effective in preventing early nausea and vomiting in children following strabismus surgery. However, the efficacy of dexamethasone in late postoperative nausea and vomiting could not be demonstrated. Further studies with a large population and different doses of dexamethasone may be warranted.
