Pediatric "off-road vehicle" trauma: determinants of injury severity and type.
This study aimed to describe the determinants of the severity and type of injuries sustained by children hurt in off-road vehicle (ORV) accidents. ⋯ Further research (prospective, federal, and multi-institutional) is needed with a view to optimizing training schedules, rules, regulations, and licensing requirements for pediatric ORV riders.

Survey of trauma registry data on tourniquet use in pediatric war casualties.
Previously, we reported on the use of emergency tourniquets to stop bleeding in war casualties, but virtually all the data were from adults. Because no pediatric-specific cohort of casualties receiving emergency tourniquets existed, we aimed to fill knowledge gaps on the care and outcomes of this group by surveying data from a trauma registry to refine device designs and clinical training. ⋯ Level 4; case series, therapeutic study.

Case ReportsImported pediatric malaria presenting to an urban pediatric emergency department: a case series.
The objective of this study was to identify common presenting signs, symptoms, and laboratory findings of imported pediatric malaria presenting to a US pediatric emergency department (ED). ⋯ The presenting signs and symptoms of imported pediatric malaria are nonspecific and inconsistently present, underscoring the importance of maintaining a high index of suspicion for this diagnosis in any patient returning from a malaria-endemic region.

Case ReportsAerophagia in a toddler with gastric pull-up: an unusual combination precipitating severe respiratory distress.
Aerophagia is a common disorder in adults as well as children, which often goes unnoticed. In toddlers, it may lead to acute gastric dilatation, which, if severe enough, may lead to respiratory distress. ⋯ Literature search revealed few cases of aerophagia after dissociative anesthesia. Management of aerophagia in children is discussed, and the importance of proper clinical diagnosis to prevent unnecessary interventions in an emergency scenario is highlighted.

Admission and treatment factors associated with the duration of acidosis in children with diabetic ketoacidosis.
Our goal was to determine the factors associated with prolonged acidosis in children with diabetic ketoacidosis. ⋯ Nongap acidosis was present in many children with prolonged metabolic acidosis. We suggest that a continuous acetate or bicarbonate therapy via maintenance fluid might be beneficial in this subgroup of patients.
