Despite the numerous guidelines and treatments available for asthma, the disease remains poorly controlled in some patients, who remain symptomatic, are a considerable burden on the health system, and account for most of the hospitalizations due to asthma. Bronchial thermoplasty is a novel experimental therapeutic option that consists of delivering radiofrequency-generated heat to the airways via a catheter inserted in the bronchial tree through a flexible bronchoscope to reduce smooth muscle quantity and contractility. ⋯ Subsequently, 2 randomized clinical trials designed to evaluate the safety and efficacy of thermoplasty in patients with moderate to severe asthma with a 1-year follow-up period showed the procedure to be safe, with mostly transient adverse affects and several clinical benefits. Although results from ongoing clinical trials are still awaited, thermoplasty may become an innovative therapeutic approach to asthma.

Chest ultrasonography is a useful tool for assessing disease activity in the peripheral lung parenchyma, pleura, chest wall, diaphragm, and mediastinum. Ultrasound imaging also provides highly useful guidance in invasive diagnostic and therapeutic procedures. The main advantages of this imaging technology are the absence of ionizing radiation and the possibility of real-time bedside applications. The chief indications and limitations of chest ultrasonography and the principal sonographic signs are reviewed.

Chronic obstructive pulmonary disease (COPD) and bronchiectasias are two of the most frequent and underdiagnosed diseases of the airways. The association between these two entities can be established from different points of view. ⋯ On the other hand, recent studies have observed an association between COPD and bronchiectasias, given that more than 50% of patients with moderate-severe COPD show bronchiectasias unexplained by other causes that could provoke an excess of bronchial inflammation, as well as a higher number of exacerbations, possibly mediated by an increase in bronchial colonization-infection by potentially pathogenic microorganisms. Lastly, some physiopathologic hypotheses that remain to be demonstrated suggest a causal relation between the two diseases in which COPD, especially severe forms, would constitute a risk factor for the formation of bronchiectasias.

Review[Prone position for the treatment of acute respiratory distress syndrome: a review of current literature].
Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) have high incidence and mortality rates. Most of the recently introduced treatments have failed to improve the prognosis of patients with ALI or ARDS or to reduce mortality. ⋯ However, current evidence strongly suggests that placing ARDS patients in prone position does not improve survival or reduce the duration of mechanical ventilation. Therefore, though in clinical practice this position may improve refractory hypoxemia in patients with ARDS, there is no evidence to support its systematic use.
