E-cigarette use in the UK is increasing and we are commonly encountering patients in the ENT clinic who are e-cigarette smokers. Currently, there is no regulatory body or licensing for e-cigarettes. This means that the contents, and therefore potential carcinogenic and toxic risks, can vary greatly from product to product. Patients are starting to ask about the risks of e-cigarettes and their use in smoking cessation. ⋯ This review aimed to examine the regulations, trends, and carcinogenic and health risks of e-cigarettes, and summarise the evidence for their use in smoking cessation. It also aimed to provide a general awareness regarding the advice and information that can be safely given to patients.

Reconstruction with a vascularised flap provides the most reliable outcome, with post-operative cerebrospinal fluid leak rates of less than 5 per cent. This article aims to review and summarise the critical technical aspects of the vascularised flaps most commonly used for skull base reconstruction. ⋯ The Hadad-Bassagaisteguy nasoseptal flap is overwhelmingly favoured for reconstructing extensive defects of anterior, middle and posterior cranial base. Its pertinent technical features are described. However, it is essential to master the skills required for the various extranasal or regional vascularised flaps because each can offer a reconstructive alternative for specific patients, especially when open approaches are needed and/or intranasal vascularised flaps are not feasible.

Intra-operative bleeding diminishes visualisation during functional endoscopic sinus surgery and can cause unfavourable outcomes. Dexmedetomidine is a potent alpha-2 agonist, with sympatholytic effects. This systematic review aimed to assess whether dexmedetomidine decreases intra-operative bleeding and improves operative field quality. ⋯ Dexmedetomidine is beneficial in providing good visibility during functional endoscopic sinus surgery. Controlled hypotensive anaesthesia with this medicine decreases intra-operative bleeding and enhances surgical field quality.

Thyroglossal duct cysts and obstructive sleep apnoea: three case reports and review of the literature.
Thyroglossal duct cysts and obstructive sleep apnoea are commonly occurring medical conditions which appear to present independently in patients. However, we noted three cases where the thyroglossal duct cysts influenced the development and/or therapy of obstructive sleep apnoea. In this article, these three case studies are presented, as is a study of the association between thyroglossal duct cysts and obstructive sleep apnoea, preceded by a literature review. ⋯ To our knowledge, the association between thyroglossal duct cysts and obstructive sleep apnoea has not been addressed previously. The results indicate that the relationship is much stronger than previously thought, and further research is required to investigate the extent of the association and possible causal relations.
