Acute ventilatory failure in COPD is caused by impaired respiratory mechanics and an imbalance of capacity and load of the respiratory muscles. Continuous positive airway pressure (CPAP), pressure support ventilation (PSV), proportional assist ventilation (PAV) or controlled mechanical ventilation (CMV) are effective in unloading the respiratory pump. CPAP reduces the inspiratory pressure time product by reducing the elastic work of breathing due to intrinsic PEEP (PEEPi). ⋯ In CMV, hyperinflation should be avoided by choice of low tidal volumes and long exspiratory times (low respiratory rate, I:E ratio 1:3, 1:4). In the weaning from CMV, PSV or PAV and CPAP is used. There seems to be a beneficial effect in nocturnal intermittent nonivasive ventilation (NPPV) in chronic hypercapnic stable COPD patients with documented hypoventilation during the night, which is reversible by NPPV.

Low back pain is one of the main problems of health care, and has many causes. To prevent low back pain daily activities are highly important--static work as well as dynamic movement--carried out in an ergonomic and economical way. Studies have shown that the compliance of patients with regard to exercise training and ergonomic recommendations is not very high. ⋯ It is simple and can be undertaken everywhere. A growing number of people have become interested in Qi Gong, and train regularly and effectively. Low back pain rarely occurs in such people, and therefore Qi Gong should be considered as one of the most effective means of prevention.

Biography Historical Article[Rokitansky and the Vienna Medical School--from the philosophy of nature to natural science].
On February 19th 2004 we celebrated the 200th anniversary of Carl Rokitansky's birth. Together with Morgagni and Virchow, Rokitansky paved the way for the development of modern pathology. ⋯ Rokitansky represents a personality on the threshold of the change in medicine from a phenomenological-dominated philosophy to an evidence-based science. This jubilee essay aims at outlining this development particularly with regard to the Vienna Medical School before and after Rokitansky.
