The use of an intraosseous (IO) infusion during the anesthetic care of an 8-month-old, 5.4-kg infant with cyanotic congenital heart disease (CHD) is presented. Previous vascular access had resulted in thrombotic occlusion of the upper and lower venous systems. When intravenous access could not be achieved, an IO needle was placed and IO access was used during the surgical procedure. The role of the IO route in the perioperative period is discussed and its adverse effect profile is reviewed.

This essay examines the effects of unexpected pediatric death on anesthesiology house staff, and offers a discussion of normal and abnormal patterns of grieving. The increased incidence of substance abuse and suicide among anesthesiologists is discussed, and the relationship of stress following patient death and appropriate coping skills is explored. A blueprint for managing stress is given based on a military combat stress model, and recommendations for residency training programs are made.

A comparison of anesthesiology resident and faculty electronic evaluations before and after implementation of automated electronic reminders.
To develop an automated e-mail reminder system to contact residents and faculty regarding incomplete evaluations. DESIGN, SETTING, INTERVENTION, AND MEASUREMENTS: In the retrospective study, two 9-month periods were evaluated representing pre- and post-introduction of the automated e-mail reminder system. Data collected contained the number of evaluations completed and the rating of residents and faculty at the University of Michigan Health System in 5 different categories on a 5-point scale. ⋯ An automated e-mail reminder system implemented to contact residents and faculty regarding incomplete evaluations for residents and faculty dramatically enhanced participation in the evaluation process.
