We report on a child who suffered an atlantooccipital dislocation and survived. The patient required tracheostomy and feeding gastrostomy due to retropharyngeal swelling from a traumatic pseudomeningocele. He later underwent fusion of his occiput to C3. The complications of such an injury and the anesthetic management are discussed.

The video RIFL (rigid flexible laryngoscope) is an airway management adjunct designed to facilitate endotracheal intubation. It is a novel stylet-based laryngoscope that incorporates a complementary metal oxide sensor distal chip imaging system and real-time articulation tip. The device combines features desired in fiberoptic bronchoscopy as well as in video laryngoscopy. Four cases involving its use in difficult airway management are presented.

A case of a newborn with a double-outlet right ventricle and tracheoesophageal fistula is presented. The pathophysiology of double-outlet right ventricle and its anesthetic implications for tracheoesophageal fistula repair are discussed.

To determine the influence of profiling and incentives on anesthesiologist behavior in relation to several key indicators of performance. ⋯ Tracking and rewarding physician performance with monthly profiling and a financial incentive given to the best in a peer group improves anesthesiologist performance in several key areas.
