We analyze the potential use of ultrasound in the study of the thorax in children. The physical limitations imposed on sonography by the ventilated lung and thoracic cage are well known. ⋯ These specialists take information from physical acoustic phenomena that is not directly convertible into images of the human body, starting with the interpretation and handling of artifacts. We give a critical assessment from a radiologic viewpoint that is both necessary and important.

ReviewAdvanced functional thoracic imaging in children: from basic concepts to clinical applications.
The lungs and airways are organs involved in fairly complex body functions, including ventilation, perfusion, respiratory motion and gas exchange. Imaging evaluation of the pediatric thorax is challenging because involuntary, nonsynchronous respiratory motions and cardiac pulsations degrade image quality appreciably. The extraction of clinically useful functional information from noninvasive imaging methods has been realized even in children thanks to recent technical advancements in thoracic imaging modalities. In this article, advanced functional thoracic imaging techniques in children, focusing on CT and MRI, will be explored from basic concepts to clinical applications.

Dual-energy thoracic CT provides two key insights into lung physiology, i.e. regional perfusion and ventilation, and has been actively investigated to find clinically relevant applications since the introduction of dual-source CT. This functional information provided by dual-energy thoracic CT is supplementary because high-resolution thoracic anatomy is entirely preserved on dual-energy thoracic CT. ⋯ In this respect, dual-energy CT imaging technique is at least dose-neutral, which is a critical requirement for paediatric imaging. In this review, imaging protocols, analysis methods, clinical applications and diagnostic pitfalls of dual-energy thoracic CT for evaluating lung perfusion and ventilation in children are described.

Historically, the evaluation of the pediatric chest has been accomplished via CT and conventional radiography. Our objective is to discuss and illustrate the role of US as a non-ionizing radiation alternative in the evaluation of the pediatric chest. US is a valuable tool in the evaluation of the pediatric chest. It can be used as a first-line modality in the evaluation of superficial lumps and bumps of the chest wall, diaphragmatic motion, the thymus and pleural effusions, and it can play a valuable secondary role in evaluation of mediastinal masses and pulmonary parenchymal disease.

ReviewDiffusion MRI of the neonate brain: acquisition, processing and analysis techniques.
Diffusion MRI (dMRI) is a popular noninvasive imaging modality for the investigation of the neonate brain. It enables the assessment of white matter integrity, and is particularly suited for studying white matter maturation in the preterm and term neonate brain. Diffusion tractography allows the delineation of white matter pathways and assessment of connectivity in vivo. ⋯ Furthermore, we give an overview of available analysis techniques, ranging from voxel-based analysis of anisotropy metrics including tract-based spatial statistics (TBSS) to recently developed methods of statistical analysis addressing issues of resolving complex white matter architecture. We highlight the importance of resolving crossing fibres for tractography and outline several tractography-based techniques, including connectivity-based segmentation, the connectome and tractography mapping. These techniques provide powerful tools for the investigation of brain development and maturation.
