Randomized Controlled TrialComparison of Pulmonary Gas Exchange According to Intraoperative Ventilation Modes for Mitral Valve Repair Surgery via Thoracotomy with One-Lung Ventilation: A Randomized Controlled Trial.
Impaired pulmonary gas exchange after cardiac surgeries with cardiopulmonary bypass (CPB) often occurs, and the selection of mechanical ventilation mode, pressure-controlled ventilation (PCV) or volume-controlled ventilation (VCV), may be important for preventing hypoxia and improving oxygenation. The authors hypothesized that patients with PCV would show better oxygenation, compared with VCV, during one-lung ventilation (OLV) for mitral valve repair surgery (MVP) via thoracotomy. ⋯ PCV during OLV in patients undergoing MVP via a thoracotomy with OLV showed lower PIP compared with VCV, but this did not improve pulmonary gas exchange.

Randomized Controlled TrialA Clinical Evaluation of the ProSeal Laryngeal Mask Airway With a Coopdech Bronchial Blocker for One-lung Ventilation in Adults.
To compare the effects of one-lung ventilation (OLV) offered by ProSeal laryngeal mask airway (PLMA) or endotracheal tube (ETT) with Coopdech bronchial blocker (BB) in adult patients undergoing thoracoscopic procedures, and also to evaluate the feasibility and security of application of PLMA with Coopdech BB for OLV. ⋯ The combined use of PLMA and Coopdech BB in adult patients can achieve adequate OLV for brief thoracoscopic procedures.

Superior Vena Cava Drainage During Thoracoscopic Cardiac Surgery: Bilateral Internal Jugular Vein Sheaths Versus One Percutaneous Superior Vena Cava Cannula.
To evaluate bilateral internal jugular vein sheaths as a replacement of one percutaneous superior vena cava cannula for superior vena cava drainage during thoracoscopic cardiac surgery. ⋯ The bilateral internal jugular vein sheaths were a feasible and effective option to replace one percutaneous superior vena cava cannula during thoracoscopic cardiac surgery, with better patient satisfaction.

Observational StudyHigh-Fidelity Simulation of Lung Isolation with Double-Lumen Endotracheal Tubes and Bronchial Blockers in Anesthesiology Resident Training.
Demonstrate the feasibility of using the AirSim Bronchi airway simulator to teach residents how to manage lung isolation with double-lumen endotracheal tubes and bronchial blockers and evaluate their performance with a detailed checklist. ⋯ The authors used the AirSim Bronchi simulator in a novel simulation curriculum to teach lung-isolation techniques to anesthesiology residents and evaluated performance using a detailed checklist scoring system. This curriculum is a promising educational tool.

Observational StudyIncidence and Risk Factors Associated With Ipsilateral Shoulder Pain After Thoracic Surgery.
This study was designed to determine the incidence and risk factors associated with ipsilateral shoulder pain (ISP) after thoracic surgery and to investigate characteristics, locations, and severity of ISP. ⋯ The incidence of ISP after thoracic surgery was high and the severity of pain was significant. The thoracotomy approach and the long duration of surgery are potential risk factors for ISP.
