A Survey of the Prevalence and Impact of Reporting Guideline Endorsement in Pathology Journals.
To determine the prevalence of reporting guideline endorsement in pathology journals and to estimate the impact of guideline endorsement. ⋯ The prevalence of guideline endorsement is relatively low in pathology journals, but guideline endorsement may improve the quality of reporting.

Case ReportsLineage Switch Between B-Lymphoblastic Leukemia and Acute Myeloid Leukemia Intermediated by "Occult" Myelodysplastic Neoplasm: Two Cases of Adult Patients With Evidence of Genomic Instability and Clonal Selection by Chemotherapy.
Lineage switch occurs in rare leukemias, and the mechanism is unclear. We report two cases of B-lymphoblastic leukemia (B-ALL) relapsed as acute myeloid leukemia (AML). ⋯ Lineage switch between B-ALL and AML could be intermediated by occult MDS. A pluripotent progenitor likely undergoes neoplastic transformation, resulting in a genomically unstable clone. This leads to a repertoire of heterogeneous subclones that may be selected by chemotherapy. Lineage switch heralds a dismal clinical outcome.

Massive Transfusion Protocol: Communication Ordering Practice Survey (MTP COPS).
We sought to assess ordering practices and quality of communication during massive transfusion at US level I trauma centers. ⋯ Although CPOE-based MTP ordering is offered by nearly half of US level I trauma centers, verbal/telephone ordering is by far the preferred mechanism. Our survey identifies confusion surrounding blood component needs during MTP resuscitation as an opportunity for practice improvement.

Review Case ReportsPathology Consultation on Viscoelastic Studies of Coagulopathic Obstetrical Patients.
In obstetrics, the decision to transfuse blood components has historically been driven by traditional laboratory testing in combination with direct observation of bleeding. The adjunctive use of viscoelastic testing, including thromboelastometry and thromboelastography, has gained increasing acceptance in the clinical domain. ⋯ Additional research is needed to further focus the optimum role of viscoelastic tests in major obstetrical hemorrhage.

ReviewTransportation Cooler Interventions Reduce Plasma and RBC Product Wastage.
The rate of plasma product wastage for the United States in 2011 was approximately 1.8%. The plasma wastage rate at our institution was higher, mainly due to products returned out of temperature range from procedural areas. A process review and intervention to reduce plasma wastage was undertaken, which included modifications to our transport cooler. ⋯ Our intervention resulted in significantly decreased plasma and RBC wastage and is broadly applicable, since out-of-temperature product wastage in procedural areas is likely a significant problem at many institutions.
