Novel Utilization of Strand-Specific Reverse Transcription Polymerase Chain Reaction in Perioperative Clinical Decision Making for SARS-CoV-2 Polymerase Chain Reaction Positive Patients.
In order to prevent in-hospital transmission and potential complications related to SARS-CoV-2 in the perioperative patient, most healthcare institutions require preoperative testing for SARS-CoV-2 prior to proceeding with elective surgery. The Centers for Disease Control and Prevention (CDC) recommends a time and symptom-based duration of isolation for the presumed infectious period. ⋯ We describe the novel perioperative use of a strand-specific assay to detect minus strand ribonucleic acid (RNA) in a clinical decision-making algorithm to determine optimal timing of elective surgery after a patient tests RT-PCR positive for SARS-CoV-2. This is the first description in the literature of an attempt to further stratify patients who repeatedly test positive for SARS-CoV-2 into infectious versus non-infectious for perioperative planning.

Outcomes Following Formation of a Dedicated Pediatric Liver Transplant Anesthesia Team.
Given the complex nature of liver transplant surgery, adult centers typically use a dedicated liver transplant anesthesia team, which has improved patient outcomes. ⋯ The liver group was associated with increased early extubations, decreased ventilator days, and decreased fresh frozen plasma use.

Consistency of remifentanil concentrations in propofol-remifentanil infusions. A laboratory-based study.
There is increasing interest in two-agent single-pump intravenous infusions for anesthesia and sedation in pediatric patients. Propofol-remifentanil is one such mixture. The poor miscibility of such admixtures when remifentanil is added in very high concentrations and when the admixtures are maintained in static conditions has been demonstrated; however, these physiochemical properties have not been examined in clinically relevant concentrations or settings. ⋯ The concentration of remifentanil was 5 mcg.ml-1 +/- 0.5 mcg.ml-1 and was consistent across 57 min of infusion, and two different pediatric weight profiles.

Observational StudyCorrelation between bispectral index and patient state index in children under sevoflurane anesthesia.
Because the unanticipated arousal or hemodynamic instability during anesthesia may adversely affect the physical and emotional welfare of children, adequate management of the anesthesia depth is required. We aimed to compare Bispectral Index (BIS) and Patient State Index (PSI) in children during sevoflurane anesthesia and evaluate PSI as depth of anesthesia monitor in children aged 6 months-12 years. ⋯ There was a fair correlation between BIS and PSI in children under sevoflurane anesthesia. The use of BIS and PSI as an indicator for anesthesia depth by sevoflurane is not reliable in pediatric patients.

Analysis of the radial and ulnar arteries using ultrasound and a vascular visualization device in children.
Radial artery is the preferred site for cannulation. Recently, the ulnar artery was chosen as an alternative in adults. ⋯ The ulnar artery can be considered a promising alternative to the radial artery for facilitating arterial cannulation in children.
