Dysphagia after anterior cervical discectomy and fusion: a prospective study comparing two anterior surgical approaches.
The Smith-Robinson approach is commonly used to expose the vertebrae in anterior cervical discectomy and fusion (ACDF). Postoperative dysphagia has been frequently reported following this procedure. In this approach, surgical dissection can be carried out either lateral (LEO) or medial (MEO) to the omohyoid muscle. The purpose of this study was to compare the degree of dysphagia between the LEO and MEO groups. ⋯ The findings from this study suggest that the LEO approach should be selected if the level of surgery involves C3-C4. For C6-C7 surgery, however, a left-sided MEO approach should be used. Depending on surgeon's preference, either approach can be used if both cervical levels are involved.

Unusual association of intraspinal extramedullary teratoma with congenital scoliosis in an elderly adult: case report and literature review.
Intraspinal teratomas associated with congenital scoliosis are extremely rare, especially in an elderly adult. ⋯ Rare intraspinal teratoma should be included in the differential diagnosis of intraspinal mass, especially in patient with congenital scoliosis. Patient with mature teratoma may survive with out any symptoms in the long term. Progressing neurological deficit is a main indication for surgery. Excellent clinical outcomes could be achieved by surgical resection and dural sac decompression.

The use of beta-tricalcium phosphate and bone marrow aspirate as a bone graft substitute in posterior lumbar interbody fusion.
Due to the disadvantages of iliac crest bone and the poor bone quality of autograft gained from decompression surgery, alternative filling materials for posterior lumbar interbody fusion cages have been developed. β-Tricalcium phosphate is widely used in cages. However, data regarding the fusion rate of β-TCP assessed by computer tomography are currently not available. ⋯ The technique of PLIF using β-TCP yielded a good clinical outcome 1 year after surgery, however, a high rate of pseudoarthrosis was found in this series therefore, we do not recommend β-TCP as a bone graft substitute using the PLIF technique.

Many studies regarding spinal sagittal alignment were focused mainly on above-hip structures, not considering the knee joint. Knee-spine syndrome was proposed earlier, but the mechanism of this phenomenon has not been revealed. The aim of the study was to demonstrate how spinopelvic alignment and sagittal balance change in response to simulated knee flexion in normal non-diseased population. ⋯ The knee flexion resulted in decrease of lumbar lordosis without a significant change of pelvic posture in non-diseased population group.

The Authors illustrate the feasibility of an open biopsy and complete excision of Osteoid Osteoma involving the C2 vertebral body performed via endoscopic anterior cervical approach. ⋯ The endoscopic transcervical surgery represents an interesting option for the treatment of these diseases in difficult areas of the upper cervical spine, also minimizing soft tissue trauma and collateral damage allows patients a faster and complete return to normal function. To our knowledge this is the first report of cervical spine tumor removal using this minimally invasive approach.
