Our purpose was to evaluate the efficacy of teriparatide for posterior lumbar interbody fusion (PLIF) in osteoporotic women. ⋯ There was no significant improvement in overall fusion rate and clinical outcome in our patients after injection of teriparatide, but the teriparatide group showed faster bony union and highly improved BMD scores.

Recently, it has been demonstrated that anterior release of tight structures via a transoral approach can assist posterior distraction-reduction technique in restoring the cranio-cervical anatomy in irreducible atlantoaxial dislocations. Our aim was to evaluate the radiological and clinical outcome of anterior release and posterior instrumentation for irreducible congenital basilar invagination. ⋯ We believe that a significant number of irreducible dislocations can be anatomically reduced with this procedure thus avoiding odontoid excision. Encouraging results from this short series have given us a new perspective in dealing with these challenging problems.

Cervical spondylotic myelopathy (CSM) can lead to significant disability through a spectrum of clinical manifestations ranging from dexterity loss to more profound weakness, incontinence and paralysis. ⋯ Surgical decompressions for CSM stop the progress of symptoms at 12 months post-surgery and may result in a significant improvement of NMG in two-thirds of the patients. Changes in the T1-weighted MRI images predict worse outcomes following surgery.

The purpose of the study was to explore the impact of Modic-2 changes on the clinical outcomes of single-level anterior cervical discectomy and fusion. ⋯ Modic-2 changes at adjacent level at baseline does not affect fusion rate or clinical outcome based on functional status but could negatively impact axial symptoms intensity and ASD, compared to patients without Modic-2 changes.
