We previously reported the successful development of a computer-aided diagnosis (CAD) system for preventing retained surgical sponges with deep learning using training data, including composite and simulated radiographs. In this study, we evaluated the efficacy of the CAD system in a clinical setting. ⋯ The validation of a CAD system using deep learning in a clinical setting showed similar efficacy as during the development of the system. These results suggest that the CAD system can contribute to the establishment of a more effective protocol than the current standard practice for preventing the retention of surgical items.

This presidential address, given during the Annual Symposium of the Excelsior Surgical Society of the American College of Surgeons, explores the origins of the expeditionary surgeon. The essential traits of such a surgeon-leader are defined using examples from history and are then used to examine the leadership of Edward D Churchill during World War II as the prototypical expeditionary surgeon. In the future, identifying key military surgical leaders as expeditionary surgeons would serve our nation's interests well in preserving our fighting force on the battlefield. Consideration should be given to formally training and designating such surgical leaders for the military and other austere settings.

Musculoskeletal discomfort is widely experienced by surgeons across multiple surgical specialties. Developing technologies and new minimally invasive techniques add further complexity and ergonomic stressors. These stressors differentially affect male and female surgeons, but little is known about the role these sex disparities play in surgical ergonomic stress. We reviewed existing literature to better understand how ergonomic stress varies between male and female surgeons. ⋯ A 4-fold method is proposed to overcome ergonomic barriers, including (1) improved education on prevention and treatment of ergonomic injury for active surgeons and trainees, (2) increased departmental and institutional support for ergonomic solutions for surgeons, (3) partnerships with industry to study innovative ergonomic solutions, and (4) additional research on the nature of surgical ergonomic challenges and the differential effects of surgical ergonomics on female surgeons.
