Smad proteins are important intracellular mediators of transforming growth factor (TGF)-beta signaling. Little has been known about the specific relationship between TGF-beta and TGF-beta/Smad signaling in hypertrophic scars. ⋯ These data substantiated the model of an autocrine positive loop in hypertrophic scars pathogenesis. The authors have indicated no significant interest with commercial supporters.

Although a variety of different surgical strategies for focal axillary hyperhidrosis (FAH) have proven effective, little is known of intraoperative and postoperative histologies of different surgical methods. ⋯ Histologic distribution and density of sweat glands were comparable to previous studies. All three surgical procedures evaluated are effective in the treatment of FAH. RSE and SST techniques are associated with a higher risk of side effects and cause more extensive scarring. However, one LC patient (n=1; 6.7%) did not respond to treatment.

Fellow evaluation is required by the Accreditation Council for Graduate Medical Education (ACGME). Procedural dermatology fellowship accreditation by the ACGME began in 2003 while dermatopathology accreditation began in 1976. ⋯ Procedural fellowships are evaluating fellows as rigorously as the more established dermatopathology fellowships. Both show room for improvement because one in five programs reported not evaluating fellows and roughly half of the evaluation forms provided do not address the six ACGME core competencies.
