The increasing availability and growth rate of biomedical information, also known as 'big data', provides an opportunity for future personalized medicine programs that will significantly improve patient care. Recent advances in information technology (IT) applied to biomedicine are changing the landscape of privacy and personal information, with patients getting more control of their health information. ⋯ In this article, I will discuss the major breakthroughs achieved in combining omics and clinical health data in terms of their application to personalized medicine. I will also review the challenges associated with using big data in biomedicine and translational science.

ReviewMetabolomics and systems pharmacology: why and how to model the human metabolic network for drug discovery.
Metabolism represents the 'sharp end' of systems biology, because changes in metabolite concentrations are necessarily amplified relative to changes in the transcriptome, proteome and enzyme activities, which can be modulated by drugs. To understand such behaviour, we therefore need (and increasingly have) reliable consensus (community) models of the human metabolic network that include the important transporters. ⋯ However, even these qualitative network models are not yet complete. As our understanding improves so do we recognise more clearly the need for a systems (poly)pharmacology.

ReviewTailoring chronic pain treatments for the elderly: are we prepared for the challenge?
Chronic pain is increasingly recognized as a disease and accounts for substantial suffering and disability worldwide. The aging 'baby-boomer' generation is creating a tsunami of elderly patients (>65 years old) for global healthcare systems (between 2010 and 2030). The phenotypic expression of chronic pain in the elderly can be influenced by co-morbid diseases (e.g. diabetes, cancer, depression, Alzheimer's disease, etc.), changes in physiological competency (e.g. drug metabolism/elimination) or cognitive reserve. Will a shift in the drug discovery paradigm be required to improve efficacy, side-effects or positively impact quality of life (QoL) in the elderly with chronic pain? This review highlights a number of potential pitfalls that should be considered when delivering valued pain relief medicines tailored for the elderly.

ReviewCentral nervous system biomarkers for antiobesity drug development.
With antiobesity agents, weight loss can emerge from an array of metabolic, cognitive and behavioural changes that translate into weight change over time. In early drug development, characterising these changes can actually be more informative than simply measuring weight loss. ⋯ In this review, we examine potential biomarkers for effects on metabolism and satiety, hedonics and motivation, and eating behaviour. We also review biomarkers for early detection of neuropsychiatric adverse effects.

ReviewModulating GPR40: therapeutic promise and potential in diabetes.
The class A G-protein-coupled receptor GPR40 is predominantly expressed in pancreatic beta cells and plays a major part in fatty acid amplification of glucose-induced insulin secretion. GPR40 agonists are being developed for the treatment of type 2 diabetes. ⋯ The pharmacology of GPR40 is only partially understood but recent findings suggest that full agonism of the receptor could, in addition to stimulating insulin release, engage the enteroinsular axis. Much remains to be discovered regarding the biology of the receptor to inform the development of GPR40-based drugs.
