To clarify the value of resection of gallbladder cancer involving the extrahepatic bile duct. ⋯ Patients with advanced gallbladder cancer with pEBI are candidates for resection when distant metastases are absent and R0 resection is achievable. When CRAO is unnecessary, surgical resection should be aggressively planned.

Good neighbors? The effect of a level 1 trauma center on the performance of nearby level 2 trauma centers.
In this study, we sought to determine whether the proximity of a level 1 trauma center (TC) might affect the performance of a nearby level 2 TC. ⋯ Level 2 TC assume different roles depending on the local trauma system configuration. Ideally, a level 2 TC should benefit from the presence of a nearby level 1 TC through collaborations in care protocols and shared case reviews. However, these data suggest the opposite: level 2 centers in proximity to level 1 centers might perform at a lower than expected level.

Incidence and significance of Delphian node metastasis in papillary thyroid cancer.
The objective of this study was to determine the incidence and significance of Delphian node (DN) metastasis in papillary thyroid cancer. ⋯ Metastatic involvement of the DN is often associated with ETE and increased incidence of metastatic disease to the central and lateral neck compartments. If the DN is positive on intraoperative frozen section, careful evaluation of the central and lateral nodal compartments is essential.

The purpose of this study was to characterize the clinicopathological features of invasive carcinomas arising in intraductal papillary mucinous neoplasms of the pancreas (IPMN) by histological subtype of the invasive component and to compare the outcomes of these patients to a cohort of matched patients with conventional ductal pancreatic adenocarcinoma. ⋯ In this study, the colloid carcinoma histological subtype of invasive IPMN had a more statistically favorable survival outcome than the tubular subtype. Patients with invasive tubular IPMN had no statistically significant difference in survival as matched patients with conventional ductal pancreatic carcinoma.

We hypothesize that introduction of nano-silver particles to porcine-derived small intestinal submucosa (NS-PSIS) would lead to significant enhancement in antibacterial property in repairing contaminated abdominal defect. ⋯ Nano-silver particles to PSIS lead to significant enhancements in antibacterial property in vitro and in vivo without decreasing its biomechanical resistance and biocompatibility. This study provides proof of concept for the use of nano-silver modified naturally derived PSIS as an ideal scaffold for SSI prevention in the contaminated tissue-deficient repair.
