Randomized clinical trial of endovenous laser ablation versus conventional surgery for small saphenous varicose veins.
No randomized clinical trial comparing treatment options for small saphenous vein (SSV) incompetence exists, and there is no clear evidence that this axis behaves the same as the great saphenous vein after treatment. This means that the existing literature base, centered on the treatment of great saphenous vein incompetence cannot simply be extrapolated to inform the management of SSV insufficiency. This trial compares the gold standard of conventional surgery and endovenous laser ablation (EVLA) in the management of SSV incompetence. ⋯ NCT00841178.).

Validity and feasibility of the american college of surgeons colectomy composite outcome quality measure.
To develop a reliable, robust, parsimonious, risk-adjusted 30-day composite colectomy outcome measure. ⋯ It is feasible to design a measure with a composite outcome of death or serious morbidity after colon surgery that has a low burden for data collection, has substantial clinical importance, and has acceptable reliability.

Is competency assessment at the specialist level achievable? A study for the national training programme in laparoscopic colorectal surgery in England.
To develop, validate, and implement a competency assessment tool (CAT) for technical surgical performance in the context of a summative assessment process for the National Training Programme in Laparoscopic Colorectal Surgery (NTP). ⋯ The CAT can reliably assess technical performance in laparoscopic colorectal surgery. The use of CATs to judge specialist technical performance before embarking on independent practice of new procedures is achievable on a national scale and can be adapted by other specialties.

Insulin sensitivity and secretion changes after gastric bypass in normotolerant and diabetic obese subjects.
To elucidate the mechanisms of improvement/reversal of type 2 diabetes after Roux-en-Y gastric bypass (RYGB). ⋯ The large increase of ISR response to the OGTT together with the restoration of the first-phase insulin secretion in diabetic subjects might explain the reversal of type 2 diabetes after RYGB. The large incretin secretion after the oral glucose load might contribute to the increased ISR.

Risk of gastric pouch enlargement with adjustable gastric banding in premenopausal women: sex hormones may play a role.
To examine the influence of age and gender on the development of proximal gastric pouch distension (PPD) after laparoscopic adjustable gastric banding (LAGB) surgery. ⋯ Younger women are at higher risk of PPD after LAGB surgery than men and women older than 50 years. Sex hormones may play a role in predisposing to gastric stretch after surgery. These findings may apply more broadly to the gastric "restrictive" component of other bariatric procedures.
