This study aims to identify blood biomarkers of postoperative delirium. ⋯ Preoperative plasma concentrations of Tau-PT217 and Tau-PT181 were associated with postoperative delirium, with Tau-PT217 being a stronger indicator of postoperative delirium than Tau-PT181.

Early Outcomes following Implementation of a Multispecialty Geriatric Surgery Pathway.
To examine geriatric-specific outcomes following implementation of a multispecialty geriatric surgical pathway (GSP). ⋯ In our diverse patient population, implementation of a GSP led to improved geriatric-specific surgical outcomes. Future studies to examine pathway compliance would promote the identification of further interventions.

To determine the association between SAO workforce and mortality from emergent surgical and obstetric conditions within US HR Rs. ⋯ Increasing SAO workforce capacity may reduce emergent surgical and obstetric mortality in regions with high surgical mortality but diminishing returns may be seen above 60 to 80 SAO per 100,000. To investigate inpatient satisfaction with surgical resident care. ⋯ Surgical inpatients were able to recognize their resident physicians with high frequency and rated resident care highly overall, suggesting that they may serve as a willing source of feedback regarding residents' development of core competencies such as interpersonal skills, communication, professionalism, and patient care. Future work should investigate how to best incorporate patient evaluation of surgical resident care routinely into trainee assessment to support resident development.

Immediate lymphatic reconstruction (ILR) may prevent lymphedema and decrease costs. Nationwide data suggest that current relative value unit (RVU) allocation undervalues ILR, thus introducing inefficiency into breast cancer operations when coupled with ILR. The RVUs assigned to ILR should be reevaluated in order to protect patient access to this procedure.
