To evaluate the effects of a fast-track esophagectomy protocol (FTEP) on esophageal cancer patients' safety, length of hospital stay (LOS), and hospital charges. ⋯ These findings suggest that an FTEP reduces patients' LOS, perioperative morbidity, and hospital charges.

To assess statistical reliability of hospital profiling models in ACS NSQIP (American College of Surgeons' National Surgical Quality Improvement Program). ⋯ For a large proportion of ACS NSQIP Essentials models, statistical reliability is adequate for assessing surgical quality and differentiating hospital performance. The Procedure-Targeted program is evolving in terms of statistical reliability, with promising results to date. These results also argue for broader discussions of statistical reliability in performance assessments for the profession.

We evaluated the prognostic significance of circulating tumor cells (CTCs) in patients with esophageal cancer (EC). ⋯ This is the first study to report that CTCs detected by an automated immunomagnetic detection system are independent, prognostic indicators of patients' outcome in EC. Thus, implementation of CTCs may improve accuracy of preoperative staging in EC.

This study aimed to describe the implementation of preoperative geriatric assessment (GA) in patients undergoing major cancer surgery and to determine predictors of postoperative delirium. ⋯ Preoperative GA is feasible and may achieve a better understanding of older patients' perioperative risks, including delirium.

To investigate the use of coffee, caffeinated drinks, and caffeine tablets for pharmacological cognitive enhancement (CE) among surgeons. ⋯ Surgeons often use caffeinated substances to cope with fatigue and long working hours. Coffee use was more prevalent than the use of caffeinated drinks and caffeine tablets.
