Case ReportsA Difficult Route for Percutaneous Coronary Intervention: Single Coronary Artery Anomaly.
Coronary artery anomalies (CAA) are frequently asymptomatic clinical entities, associated with variable prognosis. Single coronary artery anomaly is a rare type of coronary anatomy anomaly with little data in the literature. Although prognostic data are not clear, it is thought that the course of anomalous artery is the most important determinant. ⋯ In this report, we present a case of a single coronary artery undergoing percutaneous revascularisation with an alternative technique of advancing a guidewire into the anomalous left main coronary artery and then crossing the culprit lesion in the left anterior descending artery. This report highlights the importance of catheter stabilisation during percutaneous revascularisation with a simple yet very practical solution. Case ReportsInitial Experience in Ablation of Non-coronary Cusp PVCs by using High-density Mapping System (Rhythmia).
Idiopathic premature ventricular contractions (PVCs) most often occur under or above the semi-lunar valves from the right ventricular outflow tract and less often from the left ventricular outflow tract. Radio-frequency ablation is mostly used for patients who develop left ventricular dysfunction or intolerable symptoms. ⋯ We report a case of a young man with PVCs coming from a rare site of non-coronary cusp, which was ablated successfully by using a high-density mapping system. Case ReportsDouble Peptic Ulcer Perforation due to Cumulative Effects of Post-surgery Stress and NSAIDs: A Rare Event in Surgical Practice.
Peptic ulcer disease affects a large number of people around the world. Complications occur in 10-20% of patients and perforation develops in 2-14% of the cases. It can either be in the pyloric part of the stomach or in the first part of duodenum. ⋯ This patient presented with double peptic ulcer perforation in emergency due to post-surgery stress. She had cesarean section 10 days earlier with delayed recovery. Key Words: Peptic ulcer disease, Perforation, Graham patch repair, Cesarean section.

Case ReportsSevere Asymptomatic Hyponatremia in an Elderly Man.
Hyponatremia is regarded as the most common electrolyte imbalance among hospitalised patients. Most hyponatremic conditions present with various degrees of symptoms depending on the level of sodium. Herein, we present a case of hyponatremia in a 65-year male, which was being managed as a case of diabetic nephropathy. ⋯ The simultaneous level of potassium was 2.6 mmol/L and 2.7 mmol/L, respectively. From this case, it is important to note that the level of hyponatremia does not necessarily have a direct relationship with the severity of symptoms of hyponatremia.
