Comparison of remifentanil or fentanyl administration during isoflurane anesthesia for coronary artery bypass surgery.
The combination of inhalational agents with moderate doses of fentanyl is popular in cardiac anesthesia. Remifentanil is a new opioid metabolized by non-specific esterases. The aim of our study was to assess whether remifentanil may be superior to fentanyl during isoflurane anesthesia for coronary artery surgery. ⋯ Remifentanil is safe during inhalation anesthesia for coronary artery surgery and it appears to be more effective than fentanyl in blunting hemodynamic response before the initiation of cardiopulmonary bypass surgery.

Evaluating a new method for maintaining body temperature during OPCAB and robotic procedures.
We evaluated the performance of a new temperature management system (Allon Thermowrap, MTRE, Israel) in maintaining normothermia during OPCAB (Off-Pump Coronary Artery Bypass) procedures and Zeus-robotic IMA (internal mammary artery) takedowns. ⋯ The Allon Thermowrap system significantly out-performed conventional techniques in achieving and maintaining normothermia during off-pump and robotic procedures.

Prophylactic Saccharomyces boulardii in the prevention of antibiotic-associated diarrhea: a prospective study.
Interest to probiotics for the prevention and treatment of antibiotic-associated diarrhea is increasing gradually. The most promising seems to be Saccharomyces boulardii . Using a double-blind controlled study, we investigated the preventive effect of S. boulardii on the development of antibiotic-associated diarrhea in patients under antibiotherapy but not requiring intensive care therapy. ⋯ The results implied that prophylactic use of Saccharomyces boulardii resulted in reduced, with no serious side effects, antibiotic-associated diarrhea in hospitalized patients.

Prone versus supine position in mechanically ventilated children: a pilot study.
It is known that mechanically ventilated patients in the prone position have improved oxygenation compared with those supine. We did a prospective, randomized, controlled trial to evaluate the effect of prone position during mechanical ventilation, on survival in critically ill children. ⋯ Prone position in the first few hours of ventilation significantly improves gas exchange and oxygenation, reduces the mean airway pressures required to ventilate children, and may cause significant improvement in survival. Our study protocol allowed ventilator settings to be changed as needed during ventilation.

The sciatic nerve block in emergency settings: a comparison between a new anterior and the classic lateral approaches.
Anterior and lateral approaches to the sciatic nerve block are performed with the patient in the supine position. This could be an important advantage when mobilization to the limb involved is limited or painful, particularly in emergency conditions. The aim of this prospective, randomized study was to compare these two sciatic nerve blockades performed in an emergency setting. ⋯ These data suggest that both techniques are of similar value in an emergency setting.
