Comparative StudyComparison of modern steroid-eluting epicardial and thin transvenous pacemaker leads in pediatric and congenital heart disease patients.
Optimal pacemaker lead choice in pediatric patients eligible for either epicardial or transvenous leads remains unclear. We compared performances of modern thin transvenous (TTV) and steroid-eluting epicardial (SEE) leads in patients followed at one pediatric center. ⋯ Both SEE and TTV leads showed good mid-term performance and survival in our cohort. Higher TEs seen for SEE leads, especially ventricular and unipolar leads, may result in higher current drain and thus more generator replacements than TTV systems. Lead failure rates were comparable across lead types. TTV leads offer a promising alternative to SEE systems in terms of performance for young patients without intracardiac shunting who do not require open-chest surgery for another indication.

Intracardiac Echocardiography is gaining popularity as the enabling technology for real-time intracardiac imaging. It offers the unique advantage of being able to visualize anatomy and hemodynamics while also providing the interventional cardiac electrophysiologist real-time feedback on other catheters deployed in the heart. In this mini-review, current state of the technology and applications are summarized. Evolving technologies and applications are also reviewed.

Computerized three-dimensional potential mapping with a multielectrode basket catheter can be useful for pulmonary vein electrical disconnection.
Pulmonary vein (PV) isolation (PVI) has been recently proposed as an effective technique to cure atrial fibrillation (AF). ⋯ Computerized three-dimensional potential mapping can be useful for PVI because it can not only identify the true breakthrough, but can also confirm the elimination of the breakthroughs by the change in the activation sequence through the network of the PV musculature.
