To establish whether laparoscopic incidental appendectomy in gynecological diseases is related to postoperative intraabdominal infection and complications. ⋯ Postoperative bacterial growth and complications were increased in the laparoscopic incidental appendectomy group. Infections with bacteria from the intestine in both groups were also related to contamination during surgery and postoperative care.

A checklist for timeout on labor and delivery: a pilot study to improve communication and safety.
To assess the impact on staff communication of a standardized checklist for timeout for patients undergoing a trial of labor after cesarean section and/or elective induction at term. ⋯ Implementation of a formalized obstetric timeout improved the subjective perception of communication among obstetric staff. This tool has the potential to improve patient safety in labor and delivery.

To determine whether obstetric handoff quality differs morning versus evening, weekend versus weekday, or based on provider type. ⋯ Based on criteria developed for this study, handoff quality may vary based on time of day and provider type. These findings present an opportunity to further assess reasons for variation and propose changes to standardize and improve the handoff process.

To compare the clinical responses of extremely low birth weight (ELBW) infants resuscitated in polyethylene bags with ELBW infants who were resuscitated using traditional temperature control measures. ⋯ Resuscitation of ELBW infants in polyethylene bags led to higher skin temperature on admission to the NICU and at 1 hour of life. These infants were less likely to develop grade 3-4 PVL than infants resuscitated using traditional temperature control measures. No deleterious clinical effects were observed in infants resuscitated using polyethylene bags.

Nonobstetric surgery during pregnancy is not an infrequent occurrence. Guidelines for fetal monitoring during nonobstetric surgery are limited. We describe a case of appendectomy during third trimester, complicated by in utero fetal demise (IUFD). ⋯ Continuous electronic fetal monitoring during third trimester nonobstetric surgery should be available and implemented after appropriate patient counseling. This approach reduces the risk of fetal mortality.
