To examine the frequency, nature and determinants of pharmacy compounded medicines in Dutch community pharmacies. ⋯ Based upon our research, all Dutch community pharmacies compound more than 13,000 medicines per day (2.3% of all prescriptions). They consist mainly of dermatological preparations. Younger children (< 12 yr) receive a significantly higher rate of compounded medicines than other people. At least 1.2 compounded prescriptions per pharmacy per day have a specific pharmaceutical care reason according to the pharmacists.

To describe current practices for i.v. drug admixture preparation, to identify potential improvements for the enhancement of patient safety. ⋯ This survey highlighted areas for improvement in the preparation of i.v. drugs. It may contribute to raising awareness among nurses and physicians about the risks of medication errors. This survey also helped the pharmacy department in supporting the development of pharmaceutical procedures, the development of satellite pharmacy, the set up of training sessions for i.v. preparation and the switch toward ready to use packages when these are available.

To analyse clinical pharmacists interventions in the ICU of the Penang General Hospital (Penang, Malaysia) and to assess the pharmaco-economic impact of these interventions. ⋯ Pharmacists interventions in the ICU of a Malaysian hospital resulted in significant cost savings in terms of drug expenses and can therefore be suggested as a routine practice in our hospital.

Relationship between general practitioners and pharmacists. ⋯ Pharmacists and general practitioners largely agree on the pharmacotherapeutic signalling role that a pharmacist should fulfill. A good relationship benefits the attitude of general practitioners towards the pharmacist's care-providing function.
