Is single-dose prophylactic gentamicin associated with acute kidney injury in patients undergoing cardiac surgery? A matched-pair analysis.
Although aminoglycoside treatment has been associated with nephrotoxic effects, single-dose gentamicin has been considered safe in surgery. However, the effect of a single-dose prophylactic aminoglycoside on the risk of acute kidney injury among patients undergoing cardiac surgery remains uncertain. ⋯ A single-dose prophylactic aminoglycoside in adult cardiac surgery patients was associated with an increased risk of acute kidney injury but not with a greater frequency of postoperative dialysis or mortality. No differences in the incidence of sternal infections between groups were observed.

Finding the ideal biomaterial for aortic valve repair with ex vivo porcine left heart simulator and finite element modeling.
Aortic valve (AV) repair (AVr) has become an attractive alternative to AV replacement for the correction of aortic insufficiency; however, little clinical evidence exists in determining which biomaterial at AVr would be optimal. Cusp replacement in AVr has been associated with increased long-term AVr failure. We measured the hemodynamic and biomaterial properties using an ex vivo porcine AVr model with clinically relevant biomaterials and generated a finite element model to ascertain which materials would be best suited for valve repair. ⋯ The present study has shown that postrepair left ventricular work did not increase despite a decrease in geometric orifice areas in the Hemashield and CorMatrix groups. The autologous porcine pericardium and St Jude Medical pericardial patch had the closest profile to normal AVs; therefore, either biomaterial might be best suited. Finally, the increased stresses found in the bovine pericardial patch, Hemashield, and CorMatrix groups might, after prolonged tensile exposure, be associated with late repair failure.

Tissue-engineered stent-graft integrates with aortic wall by recruiting host tissue into graft scaffold.
To prevent postoperative migration and endoleaks after endovascular aneurysm repair, we developed a tissue-engineered vascular graft that integrates with the aortic wall by recruiting the host tissue into the graft scaffold. In the present study, we assessed the mechanical properties of the new graft and evaluated the integration between the graft and aortic wall histologically and mechanically in canine models. ⋯ The PET/PGA stent-graft demonstrated histologic and mechanical integration with the native aorta. This next-generation stent-graft might reduce the risk of migration and endoleaks, leading to preferable long-term results of endovascular aneurysm repair.
