Efficacy of ketoprofen vs. ibuprofen and diclofenac: a systematic review of the literature and meta-analysis.
The aim of this systematic review of the literature and meta-analysis of randomised controlled trials (RCTs) was to compare the efficacy of orally administered ketoprofen with that of ibuprofen and/or diclofenac. ⋯ The efficacy of orally administered ketoprofen in relieving moderate-severe pain and improving functional status and general condition was significantly better than that of ibuprofen and/or diclofenac.

Booster vaccination against 2009 H1N1 influenza virus was recommended for rheumatologic patients under immunosuppressive therapy during the 2009/2010 H1N1 pandemic. In this study we assessed whether B cell depletion with rituximab influences of the antiviral immune response in 2009 H1N1 influenza virus-vaccinated patients. ⋯ In the absence of a significant effect of booster vaccinations against 2009 H1N1 influenza virus on the humoral immune response in B cell-depleted patients with autoimmune rheumatic diseases, enhanced antiviral T cell responses in patients with low B cells indicate that T cells, maybe, compensate for the impaired humoral immunity in these patients.

To identify and quantitate hypoxia inducible factor 1 alpha (HIF1 α) isoforms in circulating peripheral blood mononuclear cells (PBMCs), and to assess their effects on target gene expression in rheumatoid arthritis (RA) patients. ⋯ Even in normoxia PBMCs express stable HIF1α protein on cell activation. Whilst multiple HIF1α isoforms exist in PBMCs no differences in expression were seen in RA compared with healthy controls. RA causes constitutive adrenomedullin expression in PBMCs that is not explicable by altered HIF expression, or stabilisation. Adrenomedullin has a variety of potential biological roles in RA, including regulation of angiogenesis, and aberrant gene regulation may be relevant in RA pathogenesis.

The aim of this study was to determine the influence of leptin on the production of proinflammatory cytokines by rheumatoid synovial fibroblasts (RSFs). ⋯ Leptin may be one of the proinflammatory cytokines that up-regulates IL-6 production in RSFs via activation of JAK2/STAT3. Leptin and JAK/STAT pathway may represent a new alternative therapeutic target in the treatment of RA.

To determine the two-year outcome of patients with later-onset polyarticular rheumatoid factor (RF) negative (-) juvenile idiopathic arthritis (JIA), and predictors of outcome. ⋯ The two-year outcome of patients with late-onset RF- polyarticular JIA was very good with the majority of patients having minimally active disease at last follow-up. Presence of significant polyarthritis at presentation was the only feature associated with long-term joint activity. Sex and lab results did not show any correlation with active joint in this cohort of RF-JIA patients.
