Case Reports[A smoker with hoarseness and a swelling of his neck].
A 68-year-old man was referred to the Department of Otolaryngology because of a swelling of his neck and hoarseness. CT imaging of his neck revealed a cystic mass in the larynx as well as in the neck, with an air-fluid level. The diagnosis 'laryngopyocele' was made.

[Influence of tumour stage at breast cancer detection on survival in modern times: population based study in 173,797 patients].
To assess influence of stage at breast cancer diagnosis, tumour biology, and therapy on survival in contemporary times of better (neo-)adjuvant systemic therapy. ⋯ Tumour stage at breast cancer diagnosis influences overall survival significantly also in the current era of effective systemic therapy. Early tumour stage at breast cancer diagnosis remains vital.

[The Teratology Information Service: medicines during pregnancy and lactation].
Many women use medication during pregnancy. Both the healthcare professional and the pregnant woman often have many questions about the possible adverse effects of the medication that are not always answered in the product information. ⋯ Healthcare professionals can contact the TIS for information and individual risk assessments. In this article we discuss the background and positioning of the TIS, the characteristics of telephone consultations, the collection of data and the considerations that are important for the use of medication during pregnancy and lactation.

[When should notification of an infectious disease be mandatory? A decision aid to determine whether mandatory notification is justified].
In the Netherlands, all physicians are required to report cases of certain infectious diseases to the public health services, to allow appropriate control measures. In recent years, various requests have been submitted to add certain infectious diseases to the list of notifiable diseases. ⋯ In this article, we describe the development of this decision aid and illustrate its use in the mandatory notification application reviews for Vibrio vulnificus infection and tularaemia respectively. Based on the decision aid outcomes, mandatory notification for V. vulnificus infection was advised as not necessary whereas notification is considered mandatory for tularaemia.

Review Case Reports[Treatment of breakthrough pain in cancer patients].
Pain is common in patients with cancer (33-64%) and can be divided into background and breakthrough pain (BTP). BTP is a passing, acute pain that occurs despite the use of analgesia to control background pain. BTP may arise spontaneously or be provoked by certain movements or activities. ⋯ Both patients had to wait too long before they received their BTP medication, causing the BTP to have passed its peak. After consultation with their nurses, both patients were allowed to have one dose of breakthrough medication in advance, which resulted in better treatment of their BTP. Every hospitalized patient with BTP should have one dose of breakthrough medication ready for taking in advance.
