Randomized Controlled Trial[Treatment of patients with herpes zoster by epidural injection of steroids and local anaesthetics: less pain after 1 month, but no effect on long-term postherpetic neuralgia--a randomised trial].
To assess the effectiveness of a single epidural injection of steroids and local anaesthetics, as a supplement to the standard treatment, for the prevention ofpostherpetic neuralgia in older patients with herpes zoster. ⋯ A single epidural injection of steroids and local anaesthetics in the acute phase of herpes zoster resulted in a modest decrease in zoster-associated pain in the first month. This treatment did not, however, prevent long-term postherpetic neuralgia.

Letter Randomized Controlled Trial Clinical Trial[Treatment of stroke by reducing the body temperature; 'Paracetamol (acetaminophen) in stroke' (PAIS): start of a clinical trial].
Subfebrile temperatures and fever in acute stroke are associated with poor functional outcome. A 1 degree C rise in body temperature may double the risk of a poor outcome in patients who are admitted within 12 hours from the onset of symptoms. Two randomised double-blind clinical trials in patients with acute ischaemic stroke have shown that treatment with a daily dose of 6 g paracetamol results in a small but rapid and potentially worthwhile reduction of 0.3 degree C (95% CI: 0.1-0.5) in body temperature. ⋯ The primary outcome is death or dependency at three months. The study protocol is simple, and the amount of data to be gathered is limited. The trial will run for four years.

Randomized Controlled Trial Multicenter Study Clinical Trial[Dutch research on the effectiveness of medical prescription of heroin; background, research design and preliminary results].
In the Netherlands the total number of heroin addicts amounts to approximately 25,000. Of these about 70% is in contact with the treatment system. The remaining 30% have not been seeking help, believes that no help is needed or has lost faith in a better future altogether. ⋯ During the treatment no medical complications have been observed and no serious public order or safety problems have occurred. Study participants have been very compliant both with the treatment regimen and the research requirements. The latter is indicated by the fact that 85% of all the two-monthly assessments have been completed.

Randomized Controlled Trial Clinical Trial[Surgical treatment of carpal tunnel syndrome: endoscopic or classical (open)? A prospective randomized trial].
To compare the results of open with endoscopic release of the carpal tunnel in patients with the carpal tunnel syndrome. ⋯ Endoscopic release of the carpal tunnel is as effective as the open release but it gives less postoperative pain. Because of the risk of complications and the additional costs, the endoscopic release is not the preferred method for treatment of the carpal tunnel syndrome, however.

Randomized Controlled Trial Comparative Study Clinical Trial[Less headache following lumbar puncture with the use of an atraumatic needle; double-blind randomized study].
To investigate the difference in frequency of headache after lumbar puncture with a standard needle or an atraumatic needle. ⋯ In comparison with a standard needle, there is significantly less postpuncture headache after lumbar puncture with an atraumatic needle.
