There is a new type of injury that is observed increasingly often: frostbite of the thighs that has occurred as result of holding a cylinder containing nitrous oxide in position between the legs during recreational use of said substance. Because skin symptoms are often mild in the first few days after the event, the severity of the injury is often not recognized. However, patients can suffer serious scarring from this type of injury. ⋯ It is important to recognize this injury and to consult one of the burns units to treat these patients in a timeline and appropriate manner.

Since the introduction of the electronic e-cigarette a few years ago, its use has greatly increased. The liquid formulations used in these e-cigarettes contain nicotine in high concentrations; ingestion of these liquids can be fatal. ⋯ Nicotine e-liquids are highly concentrated. Intentional ingestion can lead to toxic levels of nicotine which are associated with cardiac arrhythmias or arrest. Because even a few millilitres can be lethal, nicotine intoxication due to e-liquid ingestion should be considered potentially life-threatening.

[Practice variation in the application of remifentanil during labour; an overview of its application in Dutch hospitals].
To survey (a) the frequency of the use of patient-controlled analgesic remifentanil-PCA during labour in the Netherlands; (b) considerations by obstetricians whether or not to offer remifentanil-PCA; (c) target population for remifentanil-PCA and (d) the application of maternal monitoring. ⋯ A large variation between Dutch hospitals exists in the application of remifentanil-PCA during labour. The most common motives mentioned by obstetricians for its use are 'a need for an alternative for epidural analgesia' and 'at the request of pregnant women'.

Review[Physical diagnosis--the Glasgow coma scale for the measurement of disturbances of consciousness].
The Glasgow coma scale (GCS) may be used for the evaluation of disturbances of consciousness. The GCS is a reliable and reproducible test that can easily be carried out at the patient's bedside. The GCS can be used to assess the severity of a head injury and to determine the prognosis of the patient. The prerequisites for a reliable determination of the GCS-score are training of inexperienced personnel, re-assessment by a physician with experience in neurology, correction for external factors and reporting in accordance with a protocol.
